East Cleveland PCN Ltd. is seeking a Clinical
Pharmacist to join an exciting and forward-thinking primary care network with
ambitions to truly transform patient care.
This is a great opportunity for a Pharmacist looking

includes support and supervision to progress in the CPPE Primary Care Pharmacy

Main duties of the job
As part of East Cleveland PCN's expanding pharmacy team, the post-holder will play a key role in developing & maintaining
clinically and cost-effective prescribing provision across the network
population.
With a passion for person-centred care, the post
holder will be responsible for the care management of patients with chronic
diseases undertaking clinical medication reviews to support proactive
management of complex polypharmacy patients.
The post-holder will also provide clinical leadership
in the delivery of person-centred medicines optimisation, supporting
standardised approaches to care across the PCN & forging strong
relationships with specialist and community pharmacists to promote system-wide
medicines optimisation & quality improvement.
The role will include supported progression through
the CPPE Primary Care Pharmacy Education Pathway & independent prescriber

regards to prescription and medication queries. They will support the repeat
prescription system, deal with acute prescription requests, and medicines
reconciliation on transfer of care and systems for safe prescribing providing
expertise in clinical medicines advice, whilst proactively and reactively addressing
both public and social care needs of the network population.
