Job summary

The Social prescribing link worker will connect people to community-based support, including activities and services that meet practical, social, and emotional needs that affect their health and wellbeing.This includes connecting people to statutory services for example housing, financial and welfare advice.

Social prescribing works particularly well for people with low level mental health needs, who feel lonely or isolated, with long term conditions and complex social needs.

Social prescribing link workers work collaboratively across the health and care system, targeting populations with greatest need and risk of health inequalities. They collaborate with partners to identify gaps in provision and support community offers to be accessible and sustainable.

Main duties of the job

The post holder will work as part of a multi-disciplinary team with a personalised care approach in a patient facing (non-clinical) role. The Social Prescribing Link Worker will work with patients in the surgery or local community venues or if appropriate the patients own home.

Wellspring Surgery is based in the vibrant and diverse community of Barton Hill. The Surgery team work together with a strong focus on inner-city General Practice. We place the patient at the heart of everything we do.

The fabulous surgery team comprises 4 GP Partners, 8 salaried GPs, nurse and pharmacy teams, minor illness, physio and podiatry, personalised care roles of Care co-ordinator and Mental Health practitioner. Alongside the clinical team we have a strong and dedicated administrative team.

Job responsibilities

The following are the core responsibilities of the Social Prescribing Link Worker. There may be, on occasion, a requirement to carry out other tasks; this will depend on factors such as workload and staffing levels

To work with patients to agree goals and actions based on their identified health needs and on what is important to them

To signpost patients to local services to enable them to improve their physical and emotional well-being, and where necessary offer additional support to enable them to access these services.

To review and evaluate implementation of the action plan over an agreed time-period

In partnership with external organisations, develop and embed integrated care plans which ensure that the needs of the patient are met

Maintain accurate patient records at all times, using GP clinical systems such as EMIS

Maintain contact with fellow Social Prescribers to share and embed best practice

Build and share directory of services, incorporating referral processes to local services / organisations

To actively promote the social prescribing service to staff and patients within using a variety of culturally aware communication methods

To work with local community and voluntary sector organisations to identify and develop projects which can accept social prescriber referrals

To support practice staff in the referral process

To train the practice team in the identification of patients suitable for social prescribing

In addition to the primary responsibilities above, the Social Prescribing Link Worker may be requested to:

Produce quarterly reports in relation to service delivery and progress

Partake in audit as directed by the audit lead

Undertake other duties which are in line with purpose of the role

Essential

  • GCSE grade 4 and above in English and Maths or equivalent

Desirable

  • Health and Wellbeing qualification
  • NVQ Level 2 in Health and Social Care or other relevant professional academic qualification

  • Strong IT skills including Microsoft Office and Outlook, and ability to learn to use GP clinical systems
