Job summary

Wellspring Surgery is recruiting a practice nurse for a minimum term of 6 months maternity cover. The ideal candidate will be experienced in all things treatment room and long term condition management would be very helpful. Hours are flexible, but we are looking for someone who can work 30-37.5 hours per week and one evening finish per week. There is flexibility in the start date from late July and by the first week in September.

Main duties of the job

Working in accordance with the UKCC Code of Conduct, the post-holder will

be responsible for the delivery of practice nursing services, working as part of the practices multidisciplinary team who provide care within their scope of practice to the patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, immunisations and screening. The post holder will be required to accurately record keep and code in EMIS.

Wellspring Surgery provides primary care services to 11,000 patients in diverse and vibrant Barton Hill and surrounding communities in inner-city Bristol. Wellspring Surgery is a core teaching, training, and research practice. The team is very friendly and focused on supporting the needs of our local community. Wellspring Surgery is an active participant in the Bristol Deep End GP Network, a member of the Bristol Inner City Primary Care Network and we work closely with our colleagues across the Inner City and East locality.

The Surgery comprises 4 GP Partners, 9 salaried GPs and a fabulous and growing multi-disciplinary team of Nursing, Clinical Practitioner, Clinical Pharmacists, Podiatry, Mental Health Practitioner, BDP, Social Prescribing and Administration.

Job description

Job responsibilities

Primary responsibilities

Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF

Therapeutic Monitoring

Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach.

  • Recognising abnormalities.
  • Identifying the impact of treatment and implementing or recommending changes as appropriate.

Wound Care and Management

  • Assessing routine wounds including trauma.
  • Assessing to support good wound management in complex situations (eg use of Doppler technique).

Patient Health Checks

  • Working with patients to develop a management plan where health problems or potential health problems are identified.
  • Identifying health trends and public health issues and working with the team and others to combat these.

Health Screening:

  • Liaising with GPs to develop health screening programmes, counselling patients on life-style changes to promote health, eg lowering cholesterol and weight reduction diets, smoking/alcohol reduction/cessation.

Immunisation (Adult and Child)

  • Working with public health initiatives ensuring the work of the team is up to date.
  • Leading the team in achieving the immunisation guidelines and targets.
  • Management and running of childhood immunisation clinics, advice, counselling and immunisations.
  • Run BCG vaccination clinic for practice patients and patients registered elsewhere

Treatment Room experience

  • Variety of injections
  • Doppler
  • Wound care (ulcer/Doppler) and dressings

Minor Surgical Procedures:

  • Assist GPs with minor surgery when required.

Contraception and sexual health, womens health

  • Taking cervical smears
  • Sexual health screening including involvement in the 4YP programme
  • Providing and advising on emergency contraception.
  • Assisting with insertion of IUD/ IUS.

Management of Chronic Disease:

  • Managing of asthmatic, COPD, diabetes, hypertensive and CHD patients in clinic situation. Development of individual management plans; agreeing these as appropriate with the patient and other health professionals. Sharing responsibility with the GPs and practice manager for meeting QOF targets.

Other core duties

Maintain accurate clinical records in conjunction with extant legislation

Ensure read codes are used effectively

Maintain chronic disease registers

Chaperone patients where necessary

Support the team in dealing with clinical emergencies

Recognise, assess and refer patients presenting with mental health needs

Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)

Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

Support the clinical team with all safeguarding matters, in accordance with local and national policies

Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

  • Deliver opportunistic health promotion where appropriate
  • Support and deliver clinical audit as required
