The Role:

Main duties of the job

We are a total triage practice using the eConsults platform.

  • You will be required to work a mixture of face to face, telephone appointments, triage and duty doctor sessions.
  • Our sessions consist of 15minute appointments
  • The triaging doctors will signpost to the most appropriate healthcare professional
  • Additional CPD (Clinical Meetings twice a month) or attendance to AHD
  • 6 weeks annual leave (plus 1 week CPD)
  • NH pension
  • Manageable workload with rare home visits

In addition, we provide a GP-led Level2 SH Service to our registered pts.

As an active member of a well-established PCN, we benefit from the support of a wide range of (ARRS) professionals, including: Social Prescriber, Health & Wellbeing Coach, Care-coordinator. MH Practitioner, Physiotherapist, Paramedic, Pharmacy Tech, Digital Transformation lead

Job Summary

As part of the practice/centre team you will provide NHS primary health care to patients registered with the practice. This includes visiting such patients at their home if and when appropriate. You are expected to participate in the health care team activities including meetings and audits, as well as taking on a role in the development of the practice. The post holder will have the opportunity to contribute to the further development of the practice.

Clinical Responsibilities

To provide clinical supervision to other clinical staff as appropriate

  • The post-holder will make themselves available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
  • Responding to medical problems presented by patients on the practice list, including history taking, examination, investigation, diagnosis, treatment and referral where appropriate.
  • The post holder will be expected to independently manage a large caseload of patients with complex, chronic physical and psychological problems. Establishing appropriate systems to manage common or chronic medical conditions
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • In consultation with patients and in line with current disease management protocols, developing care plans
  • Making patient referrals to other care providers as appropriate
  • Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
  • To work in accordance with agreed standards of care and within practice policies, procedures and protocols.
  • Recording clear and contemporaneous consultation notes to agreed standards using the agreed computerised systems for all patient information.
  • To communicate effectively with patients and carers, recognising the need to alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Centre and Practice Development

  • Attend regular inhouse clinical meetings
  • Will take responsibility for specific areas of service development (e.g. QOF clinical led area) and/or take on specialist role(s) (e.g. diabetic lead) within the site, providing appropriate leadership within the practice.
  • Adopt a strategic approach, participating in and contributing to the development of the practice
  • Will contribute to and comment on practice aims and objectives, including any business planning process as required.
  • Will identify and discuss any resource issues with Lead GP and Practice Manager in relation to any proposals or service development.
  • Will develop and discuss with the GP Lead innovative ideas for the further development of the practice and identify any opportunities for the practice.

Relationships

  • Will demonstrate effective team based approach to all relationships within the practice
  • Actively involve and inform all members of the team in any service development or clinical areas they lead on. The post holder will adopt the Broomwood Surgery model of service development by establishing multi-disciplinary action teams for any service development work.

  • Establish and maintain effective relationships with external bodies and personnel
  • To participate in local networking events and forum as appropriate
  • Will discuss any areas of concern regarding staffing with the GP lead, Practice Manager and Nurse lead.
  • To develop key relationships with other local practices and providers within SELICB. This will include identifying areas for joint collaboration e.g. with other practices or joint educational activities.
  • To establish The Broomwood Surgery as a provider of clinical excellence in the locality.

Teaching/Education

  • To become involved in the teaching or training of other members of the practice team and external trainees. This will include medical undergraduates and postgraduates, pharmacy students, nurses and non-clinical members of practice team.

  • To actively participate in the educational timetable of the practice. This will include making presentations in areas of clinical responsibility, providing updates following training undertaken and sessions with the whole team around service development.
