An exciting opportunity to work as an Advanced Care Practitioner
in Islington.
We are looking for a full time
ACP who can work autonomously to undertake a diverse range of functions both on
the phone and in person. Dealing with minor self-limiting illnesses, working
proactively in partnership with people with LTCs, liaising with colleagues from
other provider organisations and processing clinical administration related to
patient care.
We have operate a total triage approach with morning coffee
breaks the norm. Appointment lengths are flexible dependent on patient need, a
cap is applied to ensure a safe number of patients are offered care daily.

Main duties of the job
- To
provide clinical care to all patients (general medical service)
- To
provide clinical administrative care for patients (results, letters)

adhere to the highest personal and professional standards and to put individual patient care as a priority.
- Working in partnership with all members of the health care team to support the highest standard of service delivery and development.

KEY RESPONSIBILITIES:
Clinical practice
- Assess patients presenting
as acutely ill or exacerbation of long-term conditions
- Diagnose, plan, implement
and evaluate treatment/interventions and care for patients presenting with
an undifferentiated diagnosis
- Clinically examine and
assess patient needs from a physiological and psychological perspective,
and plan clinical care accordingly
