Health Village & Dearne Primary Care Network have an exciting opportunity for a registered Nurse to join the team.

This is a new and important role that will help shape and form the layout of our local healthcare offer in Rotherham.

The suitable candidate should be passionate about making a difference in primary care and enjoy working as part of a multi-disciplinary team across services.

The role will involve working across the 3 practices we support. so the candidate must be able to be flexible and adapt to different businesses

Unfortunately, we can not accept candidates who have worked in the district practices within the last 12 months. This includes:

Clifton Medical Centre, Market Surgery & St Ann's Medical Centre.

Main duties of the job

The post holder is responsible for the delivery of general practice nursing to the patients from the supporting practices. The focus of the role is both the delivery of evidence-based practice for patients presenting with a longterm condition, and the provision of preventative health care to the PCN population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures.
