Job summary

Appleby Medical Practice is looking for an ANP (Part time or Full time) to join their team. The successful candidate must be enthusiastic and forward thinking to become part of the small friendly team, with a well-established culture of holding the patient and their family at the centre of all that they do. At Appleby this is a role in which you will work with the team taking this role forward whilst further developing yourself and the practice, alongside current ANP colleagues.

Main duties of the job

To manage daily surgeries comprising of face-to-face consultations, telephone consultations and telephone triage work. This will include visiting of patients either in their own home or in a care home environment.

To develop and implement specialised treatment plans and health/ wellbeing programmes to manage patients presenting with a range of acute and chronic medical conditions.

To identify and support patients at risk of developing long-term conditions and providing specialist care.

To collaborate with GPs, nursing staff and non-clinical support staff alike.

Provide wound care and specialist clinics, collecting pathological specimens and interpreting test results as well as referring patients to secondary/specialist care.

Maintain accurate clinical records, ensuring read codes are used effectively & maintaining chronic disease registers.

Chaperone patients where necessary and intervene appropriately, supporting the team in dealing with clinical emergencies.

Be an extended and supplementary prescriber, adhering to guidance and support patients in the use of their OTC or prescribed medicines.

Contribute to practice targets (QOF etc.), complying with guidance and to liaise with external services/agencies to ensure the patient is supported appropriately.

Delegate clinical responsibilities appropriately.

Support the clinical team with all safeguarding matters.

Understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately

Essential

  • Experience of working in a multidisciplinary team in Primary and/or Secondary Care setting.
  • Experience of selecting and prescribing a wide range of medicines appropriately, including antibiotics and analgesia.
  • Provide basic and advanced life support.
  • Providing general clinical advice and wider health promotion.
  • Ensure safeguarding of vulnerable patients.

Desirable

  • Experience working in both primary and secondary care
