As a PCN we are also recruiting for a second 1 FTE role at Upton Village Surgery and will pool candidate short lists

Main duties of the job

If successful, you will work as part of a multidisciplinary team and be expected to develop a patient-facing role. You will learn to deliver structured medication reviews for complex patients who are deemed to be at risk of medication-related harm, and provide advice to patients and other healthcare professionals who have medication queries.

We seek a pharmacist who is adaptable to change, with excellent clinical skills and computer literacy. Candidates must have good organisational skills, be able to work independently yet also be a good team player.

Applications are invited from suitably qualified pharmacists with a minimum two years post-registration experience in primary care, secondary care or community pharmacy. The postholder will be expected to work towards an independent prescribing qualification if they do not have one already (funding for this will be provided) and complete the CPPE primary care pharmacy education programme (unless exempt). Mentoring and training will be provided to support the development of your skills to deliver this role.

About us

Chester East PCN is a forward-thinking network of four GP practices. Over the past year, we have established a pharmacy team providing clinical, patient-facing services across our network. We have established ourselves as key staff members in our practices. We now wish to add another enthusiastic clinical pharmacist to our team to be based from Park Medical Centre.

With just over 10,000 patients Park Medical Centre plays an active part in the care community meeting the needs of its patient population. You will work alongside the Operations Team, wider clinical team and Partners to deliver excellent patient centred healthcare. The practice clinical team in addition to the existing onsite Clinical Pharmacist is comprised of 5 GP Partners, 3 Salaried GPs, 2 GP Retainers, ANP, 2 Nurse Prescribers, Trainee Nursing Associate, an HCA and a Medical Assistant. We are an EMIS Web practice with no current out of hours commitments although this may be subject to change from April 2022.
