This is an exciting opportunity to join North Thoresby Practice under the East Lindsey Primary Care Network (PCN) on a contracted basis. While employed by the PCN, you will be an integral part of the North Thoresby Practice team, delivering high-quality patient care across a broad spectrum of health services.
Main duties of the job
Key Responsibilities:
- Minor Illness Management Assess, diagnose, and treat patients with a variety of minor illnesses.
- Community & Frailty Care Provide care to vulnerable populations, including the elderly and frail, both in-practice and within the community.
- Chronic Disease Management Deliver expert care for patients with chronic conditions.
- Medication Monitoring & Reviews Conduct medication reviews, prescribe as needed, and monitor ongoing treatments.
- Prescribing Work autonomously, with full prescribing responsibilities as part of a collaborative, multidisciplinary team.

- Master's degree level qualification in a relevant area of expertise (e.g., Advanced Clinical Practice, Nursing).
Experience
Essential
- Experience of working within a nursing team.
- Experience of dealing with vulnerable patients, including frailty and chronic conditions.
- Proven ability to assess, diagnose, and manage minor illnesses.
- Strong background in chronic disease management.
- Independent prescribing qualification.
- Experience with medication monitoring and reviews.
- Excellent communication and teamwork skills.
- Knowledge of community-based care and working collaboratively with multidisciplinary teams.
Desirable
- Experience of working in primary care.
- Experience of working in a GP practice setting.
- Familiarity with the structure and processes of a Primary Care Network (PCN).
- Experience working with multidisciplinary teams, including Pharmacists, Mental Health Nurses, and other allied health professionals.
