This is an exciting opportunity to join a forward thinking Practice, providing the highest standard of patient-centred health care.
We have a population of up to 10,000 patients and we lead the way for Primary Care in Huddersfield. Our Practice's QOF figures are consistently high and we provide scope for our Clinicians to develop special interests in Primary Care.
Main duties of the job
The successful candidate will be responsible for managing patients with long term conditions as part of a large, experienced clinical team including GPs, ACPs, Practice Nurses, Pharmacists and Health Care Assistants. We will welcome full time or part time applicants to be considered for this opportunity.
Salary will be negotiable, depending on experience.

We are a
supportive training practice with 5 GP partners, Salaried GP, ACP and 2 GP
Registrars. We have excellent supporting administrative and clinical teams
including Practice Nurses, Healthcare Assistants and Pharmacy
team. We have approximately 9,700 patients and operate from purpose built
premises. We are a member of the Tolson Care Partnership Network. We use TPP SystmOne and are high QOF achievers with a
Good CQC rating.

management of chronic diseases such as asthma and diabetes and offer a wide
variety of other medical services including antenatal and postnatal care, minor
surgery, childhood vaccinations, sexual health services and well-person
check-ups.
