Job summary

As a GP working at Avicenna Medical Practice, you will be acting within your professional boundaries to support our patients and local community.

You will provide dedicated care for patients at the practice from initial history taking, to clinical assessment and diagnosis, treatment and evaluation of care and ongoing holisitic and medical care going forward. You will be able to show safe, clinical decision-making and expert care, including assessments and diagnostic skills, for our patients. You will work together with our close Practice team to meet all patient needs and requirements, whilst supporting the delivery of policy and procedures within the practice.

You will assess, diagnose, plan, implement, and evaluate treatment plans and interventions and care for patients presenting with an undifferentiated diagnosis, both physically and psychologically.

The job will vary and you will be working with chronic conditions, sometimes acute, but at all times you will be providing the highest standards of patient care, with a highly supportive multi-skilled team around you.

We are a practice of 8800 patients, who also delivers a number of AQP contracts offering training and development opportunities for the right candidates.

Main duties of the job

You will need to be a fully qualified and registered GP, having maintained your position on a performer's list, who can assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.

Prescribe and review medication appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of practice.Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Prioritise, organise and manage own workload in a manner that maintains and promotes quality, including clearing clinical tasks on a daily basis, filing pathology results in a timely manner and re-setting recalls appropriately

Deliver care according to NSF, NICE guidelines, Practice Protocols and evidence-based care

Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation including appraisal/revalidation process

Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities

Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance

Undertake at least one full audit cycle per year on a topic that is of interest to the practice and present your findings to the team

Follow the practice formulary for prescribing and not routinely be an outlier in prescribing behaviour Prescribe in line with KPPIs

Share best practice and innovation within your immediate team and help cascade such practice throughout the whole organisation

Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role mode
