Job summary

An exciting opportunity has arisen for a motivated and enthusiastic Advanced Clinical Practitioner (ACP) to join our dynamic team at Briarwood Medical Centre.

We are seeking a highly skilled clinician to deliver high-quality, person-centred care, working autonomously and as part of our multi-disciplinary team. You will play a key role in supporting the clinical workload, including triage, assessment, diagnosis, treatment, and evaluation of patient care within your scope of practice.

Working closely with the wider team and reporting to the GP Partner and Practice Manager, you will help to ensure delivery of safe, effective, and timely care while also contributing to service improvement and clinical governance. The postholder will also support our preparations for regulatory compliance, including CQC inspection readiness.

Main duties of the job

  • Undertaking patient consultations independently, including physical examinations, diagnostic decision-making, and treatment planning.
  • Prescribing medications within your professional competence and scope of practice.
  • Supporting long-term condition management and urgent care presentations.
  • Promoting high standards of clinical care and supporting quality improvement initiatives.
  • Working collaboratively with clinicians, administrative staff, and external partners to improve outcomes and service delivery.

Clinical Practice

Undertake holistic clinical assessments, diagnoses, treatment planning and prescribing for patients presenting with undifferentiated and undiagnosed conditions.

Manage long-term conditions such as hypertension, asthma, COPD, diabetes, and heart failure in accordance with national guidelines.

Conduct medication reviews and optimise medicines use, particularly in polypharmacy, frailty, and multimorbidity.

Independently manage minor illnesses and acute presentations using evidence-based practice.

Perform clinical examinations, interpret diagnostic tests, and initiate appropriate referrals when needed.

Prescribe and deprescribe safely and appropriately within scope of practice.

Manage caseloads and triage appointments, home visits or telephone/video consultations where required.

Medicines Optimisation

Lead on repeat prescribing systems, prescription queries, and formulary compliance.

Support QOF and IIF achievement through targeted medication reviews and audits.

Identify cost-effective prescribing opportunities and liaise with local pharmacy teams.

Provide clinical leadership and mentorship to other pharmacy team members (e.g., technicians, pharmacy assistants).

Education, Mentorship and Leadership

Supervise and mentor junior clinical pharmacists and pharmacy technicians within the practice/PCN.

Support induction and upskilling of multi-disciplinary team members on medication-related topics.

Lead quality improvement projects including audits, guidelines implementation, and clinical governance activities.

Collaboration and Teamwork

Work closely with GPs, nurses, paramedics, social prescribers, and other MDT members to deliver integrated care.

Liaise with community pharmacy, secondary care and PCN pharmacy teams to ensure seamless medicines management.

Actively participate in clinical and business meetings, including clinical governance reviews and MDT discussions.
