Job summary

Briarwood Medical Practice is seeking a part-time or full time Practice Nurse who will work within their professional scope of practice providing care for patients face to face and via telephone consultations.

The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.

The post holder will use skills, knowledge and competencies as a qualified Nurse in order to be responsible and accountable for managing patient caseloads for treatments, investigations, monitoring and referrals.

Ideally aware of QOF and KPI's with previous general practice experience.

Main duties of the job

Main Purpose of Post:

To assist medical personnel in the care of practice patients including treatment, preventative care, screening and patient education.

Duties and Responsibilities of Post

  • Provide assessment, screening treatment, services and health education advice.
  • Assist with the organisation and co-ordination of the provision of nursing services for the practice.
  • Provide nursing treatment to patients in participation with general practitioners, using agreed protocols, including the taking of cervical smears.
  • Provide general and specific health screening to the practice patients (within the agreed protocols) with referral to general practitioners as necessary.
  • Be competent in administration of childhood immunisation and travel vaccination.
  • Be competent and confident in managing people with long-term health conditions
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Diagnose and manage chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

The successful candidate will be required to:

Provide a high level of clinical care for our patients managing long term conditions, duties include consulting patients for scheduled appointments

To manage daily surgeries comprising of face-to-face, telephone or video consultations.

To participate and contribute to the delivery of Quality Outcome Framework (QOF) and the GP Quality Contract.
