Main duties of the job

Review all prescription requests that are received into practice via electronic methods or in person

Process all prescription requests, referring any complex clinical queries to the duty doctor with any investigation to date

Review clinic letters and discharge summaries and identify any requests to change medication, set up these changes and pass on to duty doctor to approve. Compare drug discharge list with existing repeat medication screen

Action prescription related tasks/queries received from other team members

Identify if a medication, blood test for monitoring or annual review is necessary and follow practice processes to ensure this is managed appropriately

Awareness of medication shortages and protocols to manage prescription requests

Ensure the issuing of certain medications are being managed under Shared Care Agreements

Manage requests for tests, raising and sending the blood form for the patient if required

Work closely with other practice teams, both clinical and administrative, the PCN Pharmacy Team, local pharmacies, care and nursing homes and residential schools

Manage care home/nursing home/residential institutes repeat and interim prescription requests

Manage requests for appliances

Manage telephone queries to the Prescription Team from colleagues, Care Home, Pharmacies or patients

Use of practice systems EMIS and Docman, MS Office products

Requirement to cover team members during holiday and sickness absence

About us

Farnham Dene Medical Practice is a two-site, GMS training practice, located in the beautiful Georgian market town of Farnham, with a patient list of approximately 14,500.

We are looking to recruit a Prescription Clerk to join our busy, friendly and supportive Practice. This role is based across our two sites - within Farnham Centre for Health and at the Lower Bourne (Please note - there are limited public transport links to the Bourne, so the applicant should ideally be able to drive). The role will be 15 hours per week, working Monday, Tuesday, and Wednesday 8am to 1pm, with the potential for additional hours to cover any team absence.

The successful applicant will need to be articulate and able to communicate well. They will also need to be patient, efficient, flexible and professional with extremely good attention to detail.
