Job summary
Our nursing team at Jubilee is growing and we are looking to
recruit an experienced Advanced Nurse Practitioner to join our
established ANP team. Our ANPs work alongside GPs to provide
remote triage of patients by telephone or eConsultations, they
conduct telephone and face to face patient appointments
including history taking, clinical assessment, diagnosis, treatment
and referral where appropriate. There potentially could be some
home visiting required with this role. To apply you should be an
autonomous Practitioner within your current role. At Jubilee you
will work as part of the practice multidisciplinary team, delivering
care within your scope of practice to our patient population. You
will be based in our multidisciplinary hub within the surgery
alongside our pharmacy team, mental health, network and GP
assistant colleagues.
You must have a full masters qualification to apply for the
substantive post advertised but we will consider evidence of
masters level study and equivalent experience in the role. You
should be an autonomous clinical decision maker in your current
role.

KEY RESPONSIBILITIES
Clinical Care
Act as Advanced Nurse Practitioner, demonstrating advanced clinical
competence and a knowledge base beyond those associated with
conventional nursing roles.
Undertake consultations with patients as an autonomous practitioner and
using own clinical judgement to diagnose, treat or refer patients
Diagnose and manage both acute and chronic conditions, integrating both
drug- and nondrug-based treatment methods into a management plan
Make direct referrals to primary, secondary and social services within locally
agreed pathways, guidance and protocols.
Work with patients and their carers in order to support compliance with and
adherence to prescribed treatments
Ensure care plans are in place, including emergency health care plans
following discussion with the patient and family members
Ensure that patients receive high quality clinical care, delivered in a timely
manner.
Prescribe and review medication for therapeutic effectiveness, appropriate to
patient needs and in accordance with evidence-based practice and national
and practice protocols, within scope of practice
Ensure complete and accurate documentation of each and every patient
contact.
Work as an autonomous practitioner and as part of multi-disciplinary and multi
agency teams in order to ensure patients needs are met.
Support patients to adopt health promotion strategies that promote healthy
lifestyles, and apply principles of self-care
