Pharmacists - General Practice

Job summary

The post holder is an experienced Pharmacist who will perform Pharmacist duties as part of the clinical team. One day a week for initial period of 3 months. Pay scheme equivalent to band 7 - 8a dependant on experience

You will provide specialist Pharmaceutical advice to Medical and Nursing staff on the use of medicines and will play a key part in ensuring that medicines are used optimally

You will perform face to face medication reviews to patients with complex medications.

You will assist the team to Audit prescribing and medicine utilisation within the practice

Main duties of the job

Provide specialist pharmaceutical advice to Medical and Nursing staff on the use of medicines.

Ensure effective Medicines Optimisation in all settings.

Assist the Safety and Quality team to Audit prescribing and medicine utilisation.

Conduct Annual audits

Chadsfield Medical Practice We are passionate about providing excellent care.

We have 9 GPs (3 partners/5 salaried GPs and one Retainer), 1 F2, 1 PN, 1 TNA and 1 HCA. Also, we provide our patients with Physiotherapy services, Pharmacists, Well-being Services (mental health signposting service and counselling), Care Coordinators and a Mental Health Nurse. We have an active and supportive Practice Champion group

We achieve maximum yearly QOF points and our last CQC inspection was GOOD WITH OUTSTANDING CARE. Our Partners have roles GP Appraiser; ST1/F2 trainer; Medical Student trainer (gold award). Our excellent secretaries, receptionists, prescription clerk, administrators and apprentice work hard to support the clinicians to deliver excellent medical care.

We are situated on the edge of the Peak District, 5 minutes from J25 of the M60 and a short drive to Manchester city centre, Didsbury, Macclesfield and Buxton. Excellent rail and bus services close by. Good local schools and friendly local village atmosphere.
