Job summary

We are delighted to recruit to a new post for Clinical Operations Manager to line manage our Erewash Team Up (Aging Well) community team. This a cross organisational team looking after our housebound population both in their own home and Care Homes.

We are looking for a strong confident and motivated individual who has an interest in service improvement and will be responsive and compassionate.

You will work as part of a team with support from the Clinical Operations lead and GP Clinical lead based in Primary Care.

You will manage a mobile workforce and take responsibility for staff monitoring, lone working, rotas and adherence to policies. You will also take responsibility for data and safety quality monitoring of the Team.

Main duties of the job

On a day-to-day basis the post holder will lead and operationally manage the multi-disciplinary and multi-agency service across the PCN, and professionally coordinate the interface with Primary Care and other health and social care partners, to ensure an integrated, effective, efficient and high-quality service for service users..

To develop strong , transparent working relationships with key stakeholders, including ICB, GPs, Voluntary sector, community trust, social care and Out of hours providers .

Have strong change management andleadership skillsto support integration and service development.

About us

Erewash Health Partnership (EHP) was established on 1 April 2018 and is a single GP Partnership comprising of eight constituent practices . EHP employs approximately 250 people who work within the constituent practices and central team.

As well as providing high quality core primary care services, EHP is the prime provider for delivering services to Erewash Primary Care Network (EPCN), as well as operating its own services for EHP Partners.

Job Summary

Work in conjunction with the PCN Team Up Clinical Operations Lead and Clinical Lead to plan, develop and deliver a safe and effective service, in accordance with key national targets, clinical standards, service priorities and performance indicators as set out in the key service specifications for the service areas. The service areas are Acute Home visiting, urgent community response, enhanced health in care homes and anticipatory care.

In conjunction with PCN Team Up Clinical Operations Lead and Clinical Lead the post holder will support the development of the service, quality improvement, governance, operating procedures, data capture and reporting requirements to comply with the specific requirements of the associated national and local service specifications.

On a day-to-day basis the post holder will lead and operationally manage the multi-disciplinary and multi-agency service across the PCN, and professionally coordinate the interface with Primary Care and other health and social care partners, to ensure an integrated, effective, efficient and high-quality service for service users.

The post holder will be required to work autonomously in order to address issues impacting on service delivery with overall strategic direction from the PCN Team Up Clinical Operations Lead and Clinical Lead. The post holder will ensure that key stakeholder relationships are managed with partners across the Health and Care system.

General duties

At all times the post holder is to behave in a professional way that encourages quality care, the development of a team spirit and fosters continual improvement across the PCN.

Key Job Responsibilities

To develop the operational plan for the local service implementation, delivering the requirements of the associated national and local service specifications for Acute Home Visiting, Urgent Community Response, Enhanced Health in Care Homes and Anticipatory Care. To lead the human resource requirements of the service that include workforce planning, recruitment and training and development planning and in conjunction with the PCN manager and Service Clinical Lead develop the appraisal process for the service that build a culture of continuous improvement and ensure appropriate skills are developed and maintained.

To provide effective leadership for the service to drive the transformational change required to deliver the seamless integration for the multi-skilled and multi-agency teams.

Establish effective quality improvement and clinical governance systems and processes, under the guidance of the Clinical Lead to ensure that the service is delivered in accordance with associated clinical standards, evidenced based standards and recognised good practice e.g., NICE guidelines.

Implement recommendations from local clinical audits with support from the Clinical and Professional Leads

Participate as a lead member of the system-wide continuous improvement approach to the Ageing Well Program by attending the Derbyshire Urgent Community Response PDSA and implementation group, actively engaging in regular system-wide discussion about learnings and best practice.

Develop local policies and standard operating procedures across the service to support service delivery to required standards.

To develop the service data capture and reporting requirements to comply with the specific requirements of the associated national and local service specifications.

Ensure that national target and key local performance indicators are met in accordance with the service specification, taking appropriate action where indicators are not being met.

Analyse data and information relating to performance to inform future priorities across the portfolio.

Ensure active service user involvement in the co-design, development, and delivery of services. To implement robust processes to measures the impact service user feedback has on service improvement.

Ensure effective planning for operational delivery of the service adjusting plans in response to internal and external influences and priorities.
