Job
specification: Current NMC registration first level nurse; an accredited course in advanced practice at Batchelor or
masters level, or working towards the qualification; an accredited course in diabetes
management, Warwick Diabetes Course or equivalent; current experience and
knowledge of all aspects of the diabetes speciality; non-medical prescriber, or
working towards the qualification

to: The Partners
Role summary:
To work autonomously and
collaboratively with other members of the practice's diabetes team, to provide
specialist, evidence-based and high quality diabetes care.
Key
responsibilities
To have
clinical responsibility for a shared patient caseload; working in partnership
with patients and their families to promote high quality, patient-centred care,
this will include:
Acting as a specialist resource; providing clinical expertise,
specialist diabetes advice and support to the multi-disciplinary practice team.
Delivering evidence-based, individualised and personalised care
through holistic needs assessment, and evaluating care and all care
interventions.
Encouraging lifestyle changes to improve diabetes control
through motivational techniques.
Promoting choice by using advanced communication skills to
negotiate treatment and care options for patients, carers and their families,
whilst ensuring the delivery of high quality care.
Promoting concordance and continuity of care through the use of
non-medical prescribing.
Professional
responsibilities
Ensure NMC registration is maintained through participating in
revalidation every 3 years.
Work in accordance with the current NMC Code of Professional
Conduct.
Record accurate consultation data in patients' records in
accordance with NMC guidance.
Participate in professional development opportunities to ensure
that evidence-based knowledge and competencies are updated.
Promote evidence-based practice through the use of current
research-based guidelines.
Work collaboratively with colleagues and organisations within
and external to the practice.
Monitor effectiveness and safety of practice through clinical
supervision, peer review and use of audit.
Teaching
role:
Promote a learning environment for patients, colleagues and
other healthcare professionals.
Participate in the practice's multi-disciplinary team meetings.
Contribute to the learning of GP registrars, nursing and medical
students.
Health
and safety/risk management
The post-holder must
comply at all times with the practice's health and safety policies, in
particular by following agreed safe working procedures and reporting incidents
using the organisation's Incident reporting system.
The post-holder must
comply with the Data Protection Act (1984) and the Access to Health Records Act
(1990).

The post-holder must
comply with all policies and procedures designed to ensure equality of
employment. Co-workers, patients and
visitors must be treated equally irrespective of gender, ethnic origin, age,
disability, sexual orientation or religion.
Respect for patient confidentiality
The post-holder must
respect patient confidentiality at all times and not divulge patient
Information, unless it is in the
best interests of the patient and in accordance with the Caldicott principles.
Special
working conditions
The post-holder will have contact with
body fluids, and must seek advice as necessary on sustaining a needle-stick injury
or clearing up body fluids.
Rehabilitation of offenders act 1994
Because of the nature of the work, this
post is exempt from the provisions of Section 4(2) of the Rehabilitation of
Offenders Act 1994 (Exceptions) Order 1995.

could result in disciplinary action or dismissal by the Practice. Any information given will be completely
