Job summary
We are seeking a dedicated and compassionate Palliative Care Nurse to join our primary care team. The successful candidate will work autonomously to manage a caseload of patients, develop personalised care plans, and liaise with various external stakeholders to ensure comprehensive and holistic care. This role requires a high level of clinical expertise, empathy, and excellent communication skills.
