Job summary
Due to colleague relocation, we are looking for a
8 session GP to join our two site practice covering 6,500 patients in beautiful
North Yorkshire. We are a Systmone practice. Our main site is located in
Catterick Village with our branch surgery 5 minutes' drive away in Colburn.
Our team currently comprises
of GP partners, salaried GPs, Clinical Pharmacist, Nurse Manger, Practice Nurse
and 2 Healthcare Assistants. We also have an excellent, well-led admin team
headed up by our long service PM.
We are a well-established
training practice and have close long-term ties with the Northallerton GP
training scheme. We have hosted FY2 GP placements for over 10 years.
Training and Collaborative
Working form the heart of our Practice. We dont work a traditional on call
system choosing instead to share the daily workload equally and in close communication.
Our surgeries comprise a mix of telephone triage, telephone consultation and
face-to-face appointments. We support one another through the day and debrief
together daily.
We
would be keen to offer informal visits either remotely or in person and would ideally like the right candidate to start in January 2024

Provide high quality patient care including prevention, screening, treatment, management of chronic conditions and health promotion
Develop care and treatment plans in consultation with patients and in line with current Practice protocols as well as wider guidance e.g. NICE
Carry out surgery, telephone and video consultations and undertake home visits
Speak and see patients promptly and in a timely manner. In order to minimise waiting times.
Ensure appropriate follow up of patients
Generate, check and sign acute and repeat prescriptions in-line with practice policy
Transfer patients, where appropriate, to the repeat dispensing system
Liaise with other team members and organisations involved in patient care and respond promptly to queries
Investigate and refer patients as appropriate in line with current guidance
Deal promptly with results and correspondence, clearly communicating any actions that are required
Provide cover as needed, for all of the above, when other GPs are on leave
When providing care ensure the judicious use of resources and NHS funds
Keep clear and contemporaneous computerised consultation notes and patient records to agreed standards and in-line with practice policies
Collect data for audit and QoF purposes
Prescribe generically whenever appropriate and in accordance with the Practice formulary, commissioning and national guidance
Maintain high levels of communication with patients and carers to ensure effective delivery of care

Attend and demonstrate commitment to, clinical, clinical governance and educational meetings, ensure that accurate records and minutes are kept and circulated and that action points from previous meetings have been addressed
Show responsiveness to other doctors and team members working with you at any given time with regard to sharing of workload
Assist with the training of other staff members and also with the education of doctors and nurses in training as appropriate
Develop and promote positive collaborative working relationships with partners and all team members
Liaise with all relevant agencies and services to promote knowledge and awareness of services in the practice and local community
Complete patient reports within a timely manner
Take responsibility for own personal development and participate fully in the GP appraisal scheme
Raise with management if you identify concerns about the health or performance of a colleague or team member
Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance
