Due to retirement the surgery will have 53 Hours available in total per week to split between two positions, Full time and Part time applicants will be considered.
Essential
- Previous experience as a Practice Nurse
- Diabetes management or Respiratory management qualifications.

The Surgery @ Wheatbridge was purpose built in 2008 and has an established local team of doctors, dedicated to the health and well-being of the community that we serve.
We are committed to training and development as a training practice we regularly have a number of GP Registrars and Medical Students at different stages of their training.
We also have a team of experienced Nurses, Health Care Assistants, Administration Staff and Receptionists who are there to provide a friendly and efficient service to our patients.
Our current Nursing Team consists of:
Nurse Manager, 4 practice nurses, 2 HCA's and a Phlebotmist

Nurse Job Description
Title: Practice Nurse
Scale: Depending on experience
Accountable to: The
Partners
Job Summary
To
deliver general practice nursing services to the practice population, including
treatment, screening and advice within professional competence of the post
holder. The post holder will work in accordance with the UKCC code of conduct
and within their own professional competencies.
Key Responsibilities
Clinical Practice
To
assess the nursing needs of patients and provide appropriate care/ treatment as
appropriate according to practice policy and clinical guidelines
To
include:
- To provide chronic
disease management clinics for patient with IHD/ Hypertension/ CKD/
CVA/TIA, Diabetes and heart failure.
- Lifestyle advice / health
promotion including BP measurement,
smoking cessation advice, dietary advice and exercise promotion
- Anticoagulation
management.
- Assessing requirements
of travelers/ offering travel advice and administering travel immunisations
- Childhood vaccinations
- Simple wound
management
- Administering other
vaccinations
- Performing cervical
cytology
- Performing vaginal
examination/ taking vaginal swabs
- Family planning
advice.
- Taking blood samples
when necessary
- Administration of
intramuscular or subcutaneous medication
- Act as a chaperone for
GP physical examinations if required
- Auditing and
monitoring of care of long term conditions
Record keeping/ clinical Governance
responsibilities
- Maintain accurate,
contemporaneous record on practice computer system for all patient
contacts
- Participate in
maintenance of clinical governance systems and processes across the
practice
- Communicate with other
members of PHCT and outside agencies to plan and provide care which meets
patients needs
- Apply infection
control measures within the practice according to guidelines
- Work within practice
guidelines for safeguarding children and adults
- Monitor work areas and
practices to ensure they comply with health and safety legislation
Learning and development
- Assess own learning
needs, produce an annual personal learning plan and undertake learning as
appropriate
- Meet requirements for
nurse revalidation
- Act as a mentor for
students, assessing competence against standards as requested
- Disseminate learning
and information to other team members to share good practice and inform
others
- Participate in
mandatory training cervical cytology/ immunisations/ safeguarding/
infection control/ confidentiality / data protection/ fire safety/
equality and Diversity
