Job summary
The role of the PCN Project Support Officer will support various services and contractual requirements run within the PCN, including specific project work, care coordination and standing administrative responsibilities. They will also assist the PCN Management Team in maintaining robust data collection and reporting processes and ensure compliance with relevant procedures and policies.
Please note that this job requires travel between all 5 PCN practices and working at multiple sites.

Main duties of the job
Project
Support
Provide
project support for existing and future projects of LCS PCN
Support
the Enhanced Access Service
Develop
and run searches on SystmOne clinical system
Undertake
data analysis and assist with a range of services evaluation monitoring
Engage
with relevant clinicians for the services and stakeholder practices of the PCN
Support
the proactive identification and management of patient caseloads in line with
the PCN DES
Working
collaboratively with colleagues across the PCN, Health, Social Care and VCSE
sectors providing patient centred, integrated care.
Assist
the PCN Management Team with collation and production of project and service
documentation.
Develop
and maintain databases and systems for monitoring and evaluation
Engage
with the PCN practices to present findings, recommendations and to deliver
actions agreed
Support
training of staff as appropriate to improve service outcomes
Administration
Tasks
Support
the administrative tasks of services and PCN DES contract delivery that
includes appointment and referral management for patients
Provide
administrative support to the PCN Board, including agenda setting, room
booking, informing people of their follow-up action points
Support
the PCN Management Team in the day to day running of the PCN, ensuring
compliance with relevant policies and procedures
Responsible
for developing and maintaining systems for storage and retrieval of
information, ensuring information is accessible to other members of the PCN

the PCN Project Support Officer will comply with all information management and
information technology protocols and systems.
Location
This role will require regular travel amongst the PCN member practices
and business meeting locations within the LCS locality.
