We are looking for a highly motivated and adaptable Pharmacy Technician to join our friendly and enthusiastic team at Queensway Medical Centre. This is an exciting opportunity to become part of a forward-thinking practice working in our Medicine Management team alongside our highly skilled team of: Doctors, Clinical Practitioners, Advanced Nurse Practitioner, Physiotherapist, Mental Health Practitioner, and dedicated Nurse Chronic Disease management team.
The post ofPharmacy Technicianis an integral part of the general practice team, working within professional boundaries in general practice.
This role will work under the supervision of the Clinical Pharmacist to ensure the safe, accurate and timely supply of prescribed medication to patients.
Main duties of the job

This role
will work under the supervision of the Clinical Pharmacist to ensure the safe,
accurate and timely supply of prescribed medication to patients.
They will improve
patients health outcomes and the efficiency of primary care team by providing
direct, accessible and timely medicines an expertise including face to face
contact with patients
They will
provide technical and administrative support to the Clinical Pharmacist and
clinicians.
They will be an
integral part of the general practice team as well as part of a wider
pharmacist network and multi-disciplinary team. This includes being a conduit
of of patient medicines information in and out of local hospitals. They will
work to optimise medication issues to improve patient care and safety and
support of clinical staff in the management of patients.
Be responsible for the
care management of patients with chronic diseases and undertake clinical
medication reviews.
Through structured
medication reviews, support patients to take their medications to get the best
from them, reduce waste and promote self-care.
The Pharmacy Technician will support the
practice clinical teams with queries regarding medication from patients.

Job responsibilities
The
Pharmacy Technician is to assume the following key responsibilities in
delivering health services.
Undertake
patient facing and patient supporting roles to ensure effectivemedicines
use through shared decision-making conversations withpatients
Conduct
medicine optimisation tasks including effective medicineadministration
(e.g., checking inhaler technique), support medicationreviews
and medicines reconciliation. When required, utilise consultationskills
to work in partnership with patients to ensure they use themmedicines
effectively
Support,
as determined by the Practice, medication reviews and medicinesreconciliation
for new care home patients and synchronise medicines forpatient
transfers between care settings and linking with local communitypharmacists
Provide
specialist expertise, when competent, to address both the public health
and social care needs of patients including lifestyle advice, service information
and help in tackling local health inequalities.
Maintain
a central role in the clinical aspects of shared care protocols and liaise with
specialist pharmacists for more complex patients.
Support
initiatives for antimicrobial stewardship to reduce inappropriate antibiotic
prescribing
Assist
in the delivery of medicines optimisation and management incentive schemes
such as cost-effective prescribing and patient safety audits
Support
the implementation of national prescribing policies and guidance within GP
practices, care homes and other primary care settings. This will be achieved
through undertaking clinical audits (e.g., use of antibiotics), supporting
quality improvement measures and contributing to the Quality and Outcomes
Framework and enhanced services.
Attend
a formal appraisal with your manager at least every 12 months. Once a
performance/training objective has been set, progress will be reviewed on a
regular basis so that new objectives can be agreed.
Contribute
to public health campaigns (e.g., COVID-19 or flu clinics) through advice or
direct care.
Maintain
a clean, tidy, effective working area at all times.
Act
upon MHRA alerts, any other patient safety alerts and public health messages
ensuring patient safety and dissemination of information throughout Practice.
The
technical and administrative responsibilities of the Pharmacy Technician:
Work
with the multi-disciplinary team to ensure efficient medicines optimisation
including implementing efficient ordering and return processes and reducing wastage.
Supervise
practice reception teams in sorting and streaming general prescription requests
so as to allow GPs and Clinical Pharmacists to review the more clinically
complex requests.
Provide
leadership for medicines optimisation systems across the practice, with a range
of services to get the best value from medicines by encouraging and
implementing electronic prescriptions, safe repeat prescribing systems and
timely monitoring and management of high-risk medicines.
Provide
training and support on the legal, safe, and secure handling of medicines,
including the implementation of the Electronic Prescription Service (EPS)
Develop
relationships with other Pharmacy Technicians, Pharmacists, and members of the
multi-disciplinary team to support the integration of the pharmacy team across
health and social care including primary care, community pharmacy, secondary care,
and mental health.
Support
the delivery of QOF, incentive schemes, QIPP and other quality or cost
effectiveness initiatives.
