Job summary

We are looking for a candidate who is enthusiastic, dedicated and can demonstrate the ability to give a Friendly, Caring and Professional Nursing Service to our 20k patients. This will include Cytology, Wound care, Removal of Sutures/Clips, Asthma and COPD Management, Diabetes Management, Childhood/DES Immunisations, requesting Pathology Tests, for example Urine Culture, Swabs, and Chronic Disease Management and evaluation of care. They will work collaboratively with the General Practice Team to meet the needs of our patients.

Main duties of the job

Main Duties

General Nursing Duties, including: Dietary advice, Blood pressure checks, Urinalysis, Health screening, Dressings, Phlebotomy, Routine injections, Contraception, Minor injuries, Routine Immunisationsand vaccinations, Including Baby Immunisations and Travel Vaccinations, Removal of sutures / Clips, Health Surveillance / General Health Advice. Provide General and Specific Health Screenings to the Practice Population (within agreed protocols) with referral to other members of the Primary and Secondary Health Care Team as necessary. Chronic Disease Clinics (Diabetes, COPD, Asthma) and Cervical Screening

We are a very well-established, highly achieving Training and Teaching practice with a fully comprehensive Clinical Team, comprising of: GPs (10) Nurses (3), Pharmacists (2), HCA's (3), Physician's Associate (1) Advanced Nurse Practitioner (1) The wider Practice Team consists of a Secretarial Team (3), Reception/Admin (10) and a Management Team of (4), that being GP Managers (2) and Non Clinical Managers (2)

Job description

Job responsibilities

Provide Primary Care Nursing Services as follows::

Urgent treatment including resuscitation, stabilisation and assisting with assessment as necessary

Assessment of the urgency and severity of presenting problems by history taking and examination before referring to the appropriate Clinician or Service. Assessment may be via telephone or face to face.

Follow up of patient test results and communication with patients where appropriate.

Undertaking treatment options and carrying out appropriate procedures and Clinical interventions including, but not limited to counselling, advising and the provision of information.

Health Screening

Phlebotomy Services as required

Acting as Chaperone if required

Home Visits where required

Provide timely Clinical and Culturally appropriate Health Services through:

Ongoing Health Education and advice, including counselling to improve health and prevent disease.

Use of recall and reminder systems and, as appropriate, referral to National Screening Programmes.

Education to assist patients to reduce or change risky and harmful lifestyle behaviour.

Family Planning Services, provision of contraceptive advice and Sexual Health Services.

Delivery of immunisation programmes for both Adults, Children and Babies

Travel Services, including Vaccinations and advice.

Working with Public Health providers in the prevention and control of communicable diseases for individuals and families and reporting to relevant Public Health Providers.

Ongoing care and support for people with Chronic and Terminal Conditions.

Health promotion to the practices registered population, linking to Public Health Programmes at a Local, Regional and National level, and where appropriate, using such programmes to target specific populations.

Ensure that all patient services are charged in accordance with appropriate protocols

The Supervision and Assessment of Student Nurses and Junior Colleagues (when fully trained to do so)

Assist with the provision of information for Practice Reporting requirements as requested.

Ensure that the Emergency Trollies are checked regularly and appropriately equipped with up to date drugs.

Ensure that all information entered on to the EMIS is accurate, proportionate, appropriate and in accordance with Practice Policy.

Attend Practice Meetings and play an active part, including the dissemination of information to the wider Nursing Team.

Under take responsibility for own professional Nursing Development and attend for regular Clinical Supervision.

Understand and agree that the Confidentiality of both Patients and other Team Members is maintained.

Comply with Health and Safety and Infection Control Policy and Guidelines .

Qualifications

Essential

  • Full NMC registration
