Location: The Manor Surgery and Kennington Health Centre, Oxford

Salary: Dependent on experience

Hours: Minimum of 3 days per week

About Us

We are a well-established GP practice in Oxford serving a patient population of 26,000 across two sites, the Manor Surgery and Kennington Health Centre. Our dedicated and multidisciplinary team is committed to delivering high-quality patient care, and we are looking for a proactive and experienced Diabetes Lead Nurse to enhance our services for patients living with diabetes.

Job Summary

We are seeking an experienced and highly motivated Diabetes Nurse to take a leading role in managing diabetes care within our practice. The successful candidate will work collaboratively with our existing Diabetes Specialist Nurse and Diabetes Lead GP to ensure our patients receive high-quality, evidence-based care. The role will involve direct patient management, education, and service development, as well as supporting and mentoring other members of the clinical team.

Key Responsibilities

• Lead and coordinate the diabetes service within the practice, ensuring high standards of care.

• Work alongside the Diabetes Specialist Nurse and Diabetes Lead GP to manage complex diabetic patients.

• Provide expert clinical advice, assessment, and treatment planning for patients with diabetes.

• Deliver structured diabetes education for patients, promoting self-management and lifestyle interventions.

• Support and mentor other healthcare professionals in diabetes care, including practice nurses, GPs and Pharmacists.

• Develop and implement evidence-based protocols and guidelines in line with national and local guidance.

• Undertake regular audits and quality improvement initiatives to enhance diabetes care.

• Contribute to the management of long-term condition clinics and provide leadership in proactive patient care.

• Engage in collaborative working with local diabetes services, secondary care, and community teams.

• Stay up to date with best practices and advances in diabetes care through continuous professional development.

Essential Requirements

• Registered Nurse with a valid NMC PIN.

• Postgraduate qualification or equivalent experience in diabetes management.

• Significant experience in managing diabetes within a primary care or community setting.

• Strong leadership and mentoring skills or a willingness to develop these

• Experience in patient education and empowerment in diabetes self-management.

• Ability to work autonomously as well as part of a multidisciplinary team.

• Excellent communication and interpersonal skills.

Desirable

• Independent prescribing qualification or willingness to work towards it.

• Experience in service development and quality improvement.

• Familiarity with primary care systems such as EMIS.
