Cranbrook Medical Practice is a forward-thinking practice serving a growing list size of 4,200 patients. We are located on the outskirts of Exeter so due to the rapidly increasing population we are excited to offer this new role for a Clinical Pharmacist for 20 hours per week to support the growing demands.
The post holder will work as part of a
multi-disciplinary team in a patient-facing role. You will provide
primary support to general practice staff with regards to prescription and
medication queries and will help support and develop the repeat prescriptions
system, deal with acute prescription requests and medicines reconciliation on
transfer of care and systems for safer prescribing.
The role is pivotal to improving
the quality of care and operational efficiencies so requires motivation and
passion to deliver excellent service within general practice.
Main duties of the job
Initial Responsibilities:
- Medication Monitoring
- Answer medicine‐related enquiries from GPs, practice staff, healthcare teams (e.g. community pharmacy) and patients.
- Reconcile medicines following discharge from hospitals
- Lead within the practice to implement the new QoF QI.
- Undertake clinical medication reviews
- With the support of the prescribing lead create a schedule of practice prescribing audits and take responsibility for running them.

About us
Cranbrook Medical Practice is part of the Outer Exeter Primary Care Network, made up of 4 other surgeries located along the outskirts of Exeter.
We are (currently) a small team of one GP Partner and one ANP Nurse Partner. We also have one HCA, a nurse, another ANP, and support from our PCN with other roles. Education and support is one of our many passions and we currently support medical student training placements. We also strongly believe in supporting all team members to expand their knowledge and skills to fulfil their potential, which in turn will benefit our growing population through the services and care we provide. We will therefore ensure the sucessful candidate is supported to actively develop and manage prescribing services, helping to maximise patient safety and the efficiency of prescribing within the practice.

As an experienced pharmacist, the post holder, within their professional boundaries, will work as part of a multi-disciplinary team within the practice in a patient-facing and back office role.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. Essential
- PROFESSIONAL REGISTRATION
- Have mandatory registration with General Pharmaceutical Council (GPhC) which includes revalidation
- QUALIFICATIONS
- Hold an independent prescribing qualification, will be working towards qualification or is prepared to work towards this qualification i.e Masters degree in pharmacy (MPharm) (or equivalent).
- SKILLS, KNOWLEDGE AND EXPERIENCE
- Have experience/awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
- Have a minimum of 5 years post graduate experience in pharmacy practice, as demonstrated within a practice portfolio.
- Have an in depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- Have an appreciation of the nature of GPs and general practice and an appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
- Have excellent interpersonal, influencing and negotiating skills.
- Possess excellent written and verbal communication skills.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Possess good IT skills.
- Be able to obtain and analyse complex technical information
- Recognise priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
