Job summary

We are recruiting a Pharmacy Technician to work across our primary care network sites which includes, Brocklebank Practice (SW18), St Pauls Cottage Surgery (SW19) and The Haider Practice(SW11). We are a dynamic training practice who recognises our Pharmacists to be pivotal to good patient care. We wish to now introduce Pharmacy Technicians into the team. Previous GP experience is desirable, but not essential.

The successful candidate will be well supported through shadowing, robust clinical supervision arrangements and clinical Pharmacists team meetings to facilitate shared learning and good practice. This role will support excellent prescribing for all patients. The post holder is a Pharmacy Technician and is an integral part of the General Practice team, working within their professional boundaries and will be based on one site only.

Main duties of the job

This role will work under the supervision of the Pharmacists team to ensure the safe, accurate and timely supply of prescribed medication to patients.

The Pharmacy technician

will provide technical and administrative support to the clinical pharmacists and clinicians.

will support the PCN and practice clinical team with patients queries around medication

may be asked to complete audits around patient safety in the prescribing of medication and present the information for sharing with the clinical team.

will work with the Pharmacists team to ensure safe monitoring of medication.

We team up for peer support and education, with common back-office systems and team members who work across or in more than one practice. In this way, we achieve more together than we could separately.

We provide structured induction and mentoring in a supportive environment and with exposure to a broad spectrum of clinical work and innovative practice. There will be opportunity to work with our other clinical pharmacists and to have contact with other pharmacists working within Wandsworth Borough.

Job Responsibilities

The following are core responsibilities of the PCN Pharmacy Technician. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

To work as part of multi-disciplinary team whilst in a supporting function to the clinical pharmacists and other clinicians and within agreed prescribing standard operating procedures.

To support the clinical pharmacists as the Practice point of contact for all medicines-related matters.

To process repeated medication requests, including the conversion of acute requests to repeat medicines, and changing quantities of medicines

To issue prescriptions on line with agreed clinical protocols and encourage cost-effective prescribing throughout the PCN

To undertake medicines reconciliations post-discharge and on other transfers of care.

To update and maintain accurate and timely patient medication records on each practice clinical system, including advice given and action taken

To liaise with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care

To monitor Practice adherence to the prescribing policy

To support virtual and remote models of consultation and support including e-consultations

To support the clinical pharmacists in improving the quality and effectiveness of prescribing through clinical audit

Safety

Report medicines related incidents, contribute to investigations and root cause analyses

Participate in serious incident investigations and multidisciplinary case reviews

Report Adverse Drug Reactions and promote reporting by MDT

Oversee use and monitoring of high risk drugs and drugs of abuse

Action and respond to Drug Safety Alerts and other safety cascades.
