Job summary
We are very pleased to be advertising this part time band 5/6 Practice Nurse post. The post is 20 hours over 3 days, Wed, Thurs and Fri. We are looking for someone with prior experience of working in Primary Care and skilled in chronic disease management, vaccination programmes and cytology screening. We are looking for enthusiasm, professionalism and a willingness to develop in the practice team and to undertake any training that may be required in order to fulfil this role. You will be required to make home visits and therefore need your own transport. Salary is dependent upon previous experience. We do not follow agenda for change. Main duties of the job
The post holder will need to have very good communication skills and to be warm and friendly. You need to have good knowledge and experience of the management of all chronic diseases. You will also need to have prior knowledge and experience of the routine childhood immunisation schedule, holiday vaccinations and others that are routinely administered in Primary Care. You need to be a trained cervical sample taker. Training for any of the above can be provided for the right candidate. You will need to undertake home visits to our housebound population in your own transport. A knowledge of Emis web would be beneficial.
About us
We have a stable, established reception, admin and clinical team. There are 5 GP's an ANP and 2 part time Practice Nurses. We do not follow the agenda for change pay scale but there is an annual pay review. We work from the South Liverpool NHS Treatment Centre which is purpose built to deliver a number of primary care services to the residents of South Liverpool. Our registered population is resident in Garston. You will have a large, airy well equipped room. We are a very busy surgery and we deliver a high standard of professional care to our registered population. The team is very friendly and supportive. We will support you with an annual appraisal and agreed training and development plan. Your uniform will be provided.

Job Summary
The post holder will have continuing responsibility for the management of Chronic Disease within the practice population. They will be responsible for the continuity of care and be the lead person for communication to identified stakeholders. The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload. This will include vaccination and routine national screening programmes such as cervical cytology.
This will be done via a holistic assessment of care needs and the provision of clinical care for the patients/clients with acute and chronic highly complex needs.
Practice autonomously and demonstrate evidenced based clinical decision making.
Work within guidelines to identify and manage risk, reporting identified risks to the GP, ANP or Practice Manager.
Principle Responsibilities
Responsible for the provision of appropriate, high quality care to a defined caseload, this may be within the surgery or in the patients home.
Undertake holistic assessments of need for the patient and the family, which may be highly complex, using the appropriate model of care and evidence based practice.
Work in partnership with other professionals to enable patients/clients to be maintained in an appropriate environment, thus avoiding hospital admission.
Provide advice to patients, clients and carers on a wide range of health issues to include health promotion, life-style advice and health care.
In conjunction with the Clinical Team, develop new ways of working to provide a responsive service to patients with both planned and unplanned care needs.
Develop partnerships and joint working within other agencies as appropriate.
Maintain registration in line with The NMC.
Acts as an advocate and champion for patients and/or clients.
