Job summary

We are looking for a motivated and skilled Pharmacist to join our growing Pharmacy team to support patients centred medicines optimisation and improved management of long term conditions in primary care.

North Wood Group Practice was formed in 2017 following the merger of Tulse Hill Surgery and Crown Dale Medical Centre. The practice area extends from Tulse Hill to Upper Norwood in South-East London.

There are currently 17,000 patients registered at the Practice making it one of the largest in the London Borough of Lambeth. The local population is remarkably diverse in terms of culture, ethnicity and socioeconomic status.

The practice is within South-East London CCG, reporting to the Lambeth Borough Team.

The Partnership has undergone considerable changes. We now have four partners including 3 GPs and 1 Pharmacist to reflect the changing landscape of general practice

Main duties of the job

General

The post-holder

  •  Will follow North Wood Group Policies and Procedures such as, but not limited to: Health and Safety, Data Protection and Confidentiality.
  • Is advised that the job description describes responsibilities as they are currently required and these are subject for review and updating in the light of changing service needs. Any changes will be subject to consultation as part of the annual appraisal
  • Should be aware that job descriptions are not fixed although every effort is made to ensure that the responsibilities reflect what you are being asked to do. However this demands flexibility to meet the demands of the practice and its function.   • May be expected to work at any of the medical practice sites in line with service needs from home if needed only.

About us

The practice operates from two premises, currently the site at Crown Dale Medical Centre is closed as the building is undergoing considerable renovation which should be completed in Spring 2022. The practice is currently being operated from the Tulse Hill site. A large number of both clinical and non-clinical staff are currently working part of their week from home, partly due to Covid-19 but also due to space limitations at the Tulse Hill site.

North Wood Group Practis a busy practice and has a growing multi-disciplinary team of GPs, Nurses, Physician Associates and Trainee GPs.

There is a growing pharmacy team with Pharmacists, Pharmacy Technicians and Trainee Pharmacists in cross sector placements with Guys and St. Thomas NHS Trust, Kings College Hospital NHS Trust, South London & Maudsley NHS Trust and Community Pharmacy. The practice has a culture of education and training and numerous opportunities to develop practitioners skills.

Main Duties and Responsibilities

Policy and Service Development

  • Work as part of the wider pharmacy team to support patients/ clinician's with medication queries, reconcile medications and process medication workflow from various sources in practice.
  • Build on existing skills to manage long term conditions.
  • To work with general practice staff, on the ground, to promote and embed the role of a GP practice pharmacist and raise awareness of services available to patients.
  • Work closely with the care homes to mange complex medicines management needs and supply processes.
  • To provide general practice staff with recommendations and plan to proactively improve the quality of systems and processes to improve medicines optimisation within the general practice setting.
  • To provide training regarding medication related issues and policies/guidelines to general practice staff in order to promote medicines optimisation, repeat dispensing and high-quality cost-effective prescribing as appropriate.
  • To provide operational and clinical support to medicines optimisation developments and prescribing initiatives as dictated by GP practice, CCG, and hospital Trust and service objectives.
  • To collate and feedback on electronic discharge related information to ensure continuing improvement of the hospital computerised discharge letter and process of communication.
  • To assist with the implementation of policies and guidelines to improve medicines optimisation across primary/ secondary care interface and into the general practice setting.
  • To provide training to general practice staff to promote medicines optimisation in practice.
  • Develop and deliver patient facing services and clinics which improve patient outcomes, reflect the needs of the practice population and shift workload from other disciplines
  • Deliver high quality medication reviews and develop bespoke medicines treatment plans for patients (particularly the elderly, complex patients and those at high risk of admission) to reduce risk and unplanned hospital admissions

Patient Care

  • To act as a specialist practitioner in General Practice
  • To promote and deliver pharmaceutical care in general practice and promote safe pharmaceutical management across the interface
  • Provide pro-active leadership on medicines and prescribing systems to the practice multidisciplinary team, patients and their carers
  • To provide highly specialised evidence-based clinical advice and information to nursing and medical staff on the care and treatment of patients
  • To actively participate in key multi-disciplinary ward rounds.
  • To liaise with other pharmacists to ensure efficient transfer of clinical and medicines management issues
  • Contribute to patient safety culture through Significant Event Reporting and investigation of medicines related incidents and through pro-active measures
