Job summary
Elms Medical Practice is a local GP practice offering primary care services to over 9500 patients in Chester.
The Practice is looking to recruit a Care Navigator to our dynamic team with the day to day running of the surgery. As a Practice we will offer ongoing training and support.
As a Care Navigator you will be expected to:-
Offer general assistance to the practice team, patients and visitors to the practice, either in person or over the telephone
Receive and direct patients courteously and efficiently so that they can access the appropriate service or healthcare professional without difficulty.
Undertake various administrative duties which contribute to the smooth running of the Practice.
Ensure communication to and from patients, other members the primary care team and external agencies is processed accurately and within the necessary timescales.
Main duties of the job
The principal duties and responsibilities of a Care Navigator may include any or all of the items listed below. Individual duties may vary from time to time in relation to the prevailing workload and staffing levels, and tasking will be under the direction of the Senior Care Navigator:
Processing personal and telephone requests for appointments, visits and telephone consultations and ensuring callers are directed to the appropriate healthcare professional.
Processing and distributing incoming (and outgoing) mail.
Taking messages and passing on information.
Filing and retrieving paperwork.
Processing repeat prescriptions in accordance with practice guidelines.
Computer data entry/data allocation and collation; processing and recording information in accordance with practice procedures.
Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers.
Clearing and re-stock consulting rooms as required.
Providing clerical assistance in the form of word/data processing, filing, photocopying and scanning.
Keeping the reception area, notice-boards and leaflet dispensers tidy and free from obstructions and clutter
Any other duties as required
About us
At The Elms Medical Practice we have a clinical team of seven GPs, ANP, two practice nurses and a HCA. They are supported by a large admin team comprising of Medical Secretaries, Medicines Managers and Administrators.
We are a friendly team passionate about providing excellence in care to our patients.
We were rated "Good" at our last CQC inspection. We are high achievers in QOF and other enhanced services.
The Elms Medical Practice is part of the Chester Central Primary Care Team who provide additional health professional roles for instance pharmacists, physiotherapists, paramedics, health & well being coaches and dieticians to our team.
