LOCATION & FUNCTION
The role will be hybrid working, to deliver a Work and Health Coaching, which can include working in the GP surgeries within Leicester, Leicestershire & Rutland
This is an exciting opportunity to work in a brand new role within Primary Care General Practice, working the practices within Leicester, Leicestershire & Rutland (LLR) Primary Care Newtorks (PCN) The role will give you a unique chance to work as a member of the Care Navigation Team, whilst also building links with a range of other local services and be part of a county wide Work and Health coach network.
The successful candidate will be part of the WorkWell programme and will work with people from a range of backgrounds to help them to remain, thrive in work and return to work. They will work closely with participants within the local community to assess the barriers they experience in returning to work and provide support and coaching along with health and employment advice to overcome these barriers and return to work
This role would suit someone with previous experience of coaching and mentoring. Clinical experience is not required, and training will be provided for the successful candidate.
Please note that numerous vacancies will be advertised for this Work and Health coach role, in other Primary Care Networks (PCNs) and federations across LLR.
Main duties of the job
To manage a caseload of clients who have experienced common mental, physical and social health problems or long term conditions
To carry out a vocational assessment, to identify vocational needs and develop action plans in agreement with the individual.
Ensuring that a Participants eligibility is checked as outlined in the WorkWell Grant Guidance.
To provide an evidence based, person-centred and holistic initial assessment of barriers.
To provide a light-touch holistic support for health-related barriers to employment.
To provide a provision of a single joined-up view and pathway into local services to tackle specific need.
To ensure multiple referral pathways into and onwards from WorkWell services across the range of partners to ensure maximum opportunities for people to engage in using WorkWell services.
To provide all information/documentation requested by DWP for the purposes of activities undertaken by DWP to evaluate the effectiveness and success of WorkWell.
About us
Ripple Touch provides work and health coaching services to GP practices within the Primary Care Networks, to help support patients in the wider community.
The Work and Health coach will conduct assessments to identify the key health barriers to employment and provide health and employment advice to overcome those barriers. They will manage a caseload of people whose barriers to employment may be because of mental health, long term conditions and Musculoskeletal conditions, that have received a Fit Note by a healthcare professional. Using the WorkWell assessment plan, the post holder will provide 1:1 coaching and prescribe either multi-disciplinary in-house support or signpost and refer to external services.
