Job responsibilities

Key responsibilities will include:

To manage a caseload of clients who have experienced common mental, physical and social health problems or long term conditions.

To carry out a vocational assessment, to identify vocational needs and develop action plans in agreement with the individual.

Ensuring that a Participants eligibility is checked as outlined in the WorkWell Grant Guidance.

To provide an evidence based, person-centred and holistic initial assessment of barriers.

To provide a light-touch holistic support for health-related barriers to employment.

To provide a provision of a single joined-up view and pathway into local services to tackle specific need.

To ensure multiple referral pathways into and onwards from WorkWell services across the range of partners to ensure maximum opportunities for people to engage in using WorkWell services.

To provide all information/documentation requested by DWP for the purposes of activities undertaken by DWP to evaluate the effectiveness and success of WorkWell.

To maintain detailed and accurate case notes, action plans and WorkWell participant paperwork and upload these on to the management information system within the allocated timeframe.

To provide information, advice guidance & support to individuals about the pathways of opportunities available to them to gain, return and sustain work.

To support individuals to gain accurate benefits advice, complete paid job applications, compile CVs and to practically prepare for interviews.

To liaise and arrange meetings with health professionals, partner agencies (voluntary and statutory sectors) and/or employers concerning the individuals needs, including partner organisations that work with diverse groups to develop joint working that will enhance service delivery and increase opportunities for individuals.

To deliver training and workshops (as required) to clients on job applications, CV writing and interview skills to increase their chances of moving into employment.

To take part in informal training with colleagues from external organisations to help raise awareness and improve their knowledge about this pilot and the WorkWell model, as well as supporting them to understand the common barriers and LTCs.

To ensure that the service eligibility is adhered to, providing information on alternative employment support services for those who are not eligible, and monitoring progress and outcomes of those supported by WorkWell ensuring that stringent monitoring deadlines are adhered to, and KPI evidence is gained regularly.

To liaise and communicate regularly with the referrer and other relevant healthcare staff to ensure that all parties are kept informed and cooperate in helping the individual achieve, return and sustain employment.

To collect and collate statistics that supports the ongoing monitoring of service delivery/ quality. To provide information for service reports as required and participate in regular audits and evaluations.

To work as part of a team to further develop quality standards to continue to improve service delivery and the implementation of WorkWell.

To offer advice and guidance to the Administrator and training and support to new team members.

To work flexible hours as required to meet the needs of the individuals and the vocational service

Along with your main duties, you will also be expected to carry out any other duties that are reasonably asked of you.

Mobility

Employees may be required to work at any of the other sites within the organisation subject to consultation. The organisation is in a period of rapid change due to developments and rationalisation of services. This will lead to modification of structures and job descriptions. The post holder will be expected to co-operate with changes subject to consultation, at any time throughout the duration of your contract

Clinical Supervision
