Wolstanton Medical Centre is looking for a Nurse Practitioner to join our expanding nursing team on a part-time basis. The Nurse Practitioner will provide high-quality and responsive nursing care to our patient population.
Main duties of the job
To be responsible for the delivery of evidence-based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.
Working autonomously, the Nurse Practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Wolstanton Medical Centre within the Newcastle Central Primary Care Network (PCN) has a patient population of 11,100. The PCN are a group of 6 practices working together to focus on local patient care, led by our PCN Clinical Director and a supportive management team.
We are dedicated to providing high-quality patient care and have a long tradition of providing GP training, training medical students and student nurses working alongside Keele University.
We are a 2 GP Partner Practice, with a varied team including Salaried GPs, Advanced Nurse Practitioners, Physician Associates, Practices Nurses, and Health Care Assistants. We have a team of receptionists and secretaries ensuring the practice runs smoothly and efficiently.

Job responsibilities
The following are the core responsibilities of
the Nurse Practitioner. There may be, on occasion, a requirement to carry out
other tasks; this will be dependent upon factors such as workload and staffing
levels:
- Develop,
implement and embed health promotion and wellbeing programmes
- Develop,
implement and evaluate individual treatment plans for acute and non-acute
conditions
- Support
the clinical team in the development of individual treatment plans for chronic
disease patients
- Identify,
manage and support patients as risk of developing long-term conditions,
preventing adverse effects on the patients health
- Provide
routine nursing care to patients as required in accordance with clinical based
evidence, NICE and the NSF
- Prescribe
medication in accordance with PGD and within own scope of practice
- Review
medication for effectiveness, need and national guidance
- Request
and advice on prescribed or over-the-counter medication on side effects, common
interactions and dosage
- Support
patients to ensure compliance with prescribed medications
- Provide
wound care (ulcer/Doppler, etc.) to patients
- Provide
travel medicine services
- Request
pathology services as necessary, processing and interpreting results as
required
- Refer
patients directly to other services as appropriate
- Provide
chronic disease clinics, delivering patient care as necessary, referring
patients to secondary/specialist care as required
- Maintain
accurate clinical records in conjunction with legislation

In addition to the primary key responsibilities,
the Nurse Prescriber may be requested to:
- Act
as the audit lead, effectively utilising the audit cycle
- Monitor
and ensure the safe storage, rotation and disposal of medicaments
- Contribute
to the implementation of an effective training programme for staff and students
within the organisation
- Participate
in local initiatives to enhance service delivery and patient care
- Support
and participate in shared learning within the organisation
- Continually
review clinical practices, responding to national policies and initiatives
where appropriate
- Be
aware of and contribute to the planning and implementation of Integrated Care
System (ICS) patient pathway initiatives
- Participate
in the review of significant and near miss events applying a structured
approach i.e., root cause analysis (RCA)
