Job summary

As a Clinical Pharmacist, you will work as part of a multidisciplinary team to provide expert medication reviews, support long-term condition management, and contribute to safer, more effective prescribing across our practices. You'll play a pivotal role in improving patient outcomes through structured medication reviews, managing repeat prescribing, and supporting clinicians with medicines optimisation.

Main duties of the job

  • Conduct structured medication reviews with patients to optimise treatment and reduce risk.
  • Support management of long-term conditions through evidence-based prescribing.
  • Provide expert advice on medicines to clinicians and patients.
  • Assist in medicines reconciliation post-hospital discharge.
  • Contribute to prescribing audits, safety initiatives, and formulary adherence.
  • Work closely with GPs and the wider clinical team to enhance prescribing quality.

About us

We are excited to expand our pharmacy team and are currently recruiting for two Clinical Pharmacists and one Pharmacy Technician to join our ever-growing and dynamic workforce.

Based in Devon, with primary sites in Plymouth or Barnstaple, these roles offer the opportunity to work both on-site and remotely to support our wider network of practices across the region.

These are full-time roles (up to 37.5 hours per week), though we are happy to discuss flexible or part-time working arrangements that support work life balance.

As part of our collaborative team, you will play a key role in delivering high-quality medicines optimisation, supporting long-term condition management, and contributing to safe, effective prescribing within our primary care network (PCN).

Collaboration and Communication
