Job summary
This is a pivotal dual-role opportunity for an experienced healthcare manager to lead the operational delivery and digital transformation within a Primary Care Network (PCN). The PCN Manager will provide day-to-day leadership, ensuring that all DES and ICB priorities are delivered effectively across multiple practices.
In addition to operational management, the post holder will also act as Digital and Transformation Lead, driving innovation, service improvement, data-driven decision making, and the successful deployment of new technology and digital tools.
The role spans across business planning, workforce coordination, digital change management, stakeholder engagement, and service evaluation, with a strong focus on improving patient access, supporting staff wellbeing, and ensuring long-term sustainability of general practice services.
Main duties of the job
The post holder will work closely with the PCN Clinical Director, Administrator, GP Practice staff, such as practice managers and clinical staff to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.
The post holder will develop strong relationships with their local PCN practices and support the planning and delivery of improvements that will benefit patients and staff.
The remit of this role is broad and will require attention to detail in delivering high-quality day-to-day operations.
Summary
- Lead the operational and strategic delivery of PCN services across multiple practices.
- Coordinate workforce planning and directly support the management of ARRS roles.
- Drive digital transformation projects aligned with ICS digital and national NHS strategies.
- Utilise data to inform service development, improve access, and support quality improvement.
- Provide financial oversight, contract compliance and manage service specifications (DES).
- Engage with Clinical Directors, commissioners and wider system partners to enable integrated care.
- Support practices to implement and sustain improvements using digital tools and innovation.
- Lead governance, PCN board administration and communication across stakeholders.
About us
We are a network of four GP practices who work together to provide new and specialist services that patients in our area need.
We have the freedom to decide which services would most benefit our patients and recruit expert staff to provide those services.
In Gateshead Outer West we have a Clinical Director, Business Manager, Clinical Pharmacists, Mental Health Practitioners, Musculoskeletal Specialists and Social Prescribing Link Workers who work with patients across seven practices. It means that new and expert services are available to patients through the familiar setting of their GP Practice.

- Hollyhurst Medical Centre (IJ Healthcare)
- Oldwell Surgery
- Gateshead Healthcare (Crawcrook Medical Centre with branch practices of Rowlands Gill Medical Centre, Blaydon GP Led Surgery and Grange Road Medical Practice).
The aim is to allow neighbouring GP practices to work together to provide new and specific services that meet the changing needs of patients. Our population is growing, were living longer, and more of us take regular medication or have long-term conditions.
