Job summary

The Clinical Pharmacist is crucial for providing comprehensive pharmaceutical care, enhancing patient outcomes through effective medication management. This role involves working under the supervision of the Senior Clinical Pharmacist to review, dispense, and manage medications, ensuring their safe and effective use. This role will be based within our Organisational Pharmacy Team at Stirling road Medical Centre, but will be working for Coleridge Medical Centre which is also part of the Fuller and Forbes Healthcare Group.

Key responsibilities include conducting medication reviews to optimise therapy, managing drug interactions, providing patient counselling, and supporting the healthcare team in prescribing decisions. The Clinical Pharmacist will also participate in health promotion initiatives and contribute to the continuous improvement of pharmaceutical care practices.

This position requires a deep understanding of pharmacology, excellent communication skills, and a detail-oriented approach to patient care.

Discharge Resolutions: resolve medication issues and requests from hospital correspondence.

Pharmacy Tasks: complete the wide range of medicines-management pharmacy tasks on a daily basis.

Patient Care and Counselling:

Patient Education: Provide comprehensive counselling to patients on medication use, side effects, interactions, and storage, enhancing patient knowledge and compliance.

Clinical Decision Support: Assist healthcare providers in making informed clinical decisions regarding pharmacotherapy, utilising up-to-date clinical data and evidence-based guidelines.

Chronic Disease Management: Participate in the management of chronic diseases by providing expertise in pharmacotherapy, contributing to patient care plans, and monitoring patient progress.

Collaboration and Communication:

Multidisciplinary Team Collaboration: Work closely with a multidisciplinary team including doctors, nurses, and other healthcare professionals to ensure a coordinated approach to patient care.

Healthcare Team Support: Provide drug information and pharmacological advice to healthcare team members, supporting the safe and effective delivery of treatments.
