Advanced Nurse Practitioner

Job summary

The post holder will be an Advanced Nurse practitioner who can demonstrate the following:-

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Post registration experience in at least one of the following areas, relating to the service applying for: Out of Hours Urgent Primary care, General practice, walk in centre, A&E.
  • Be able to see patients of all ages
  • Willing to undertake predominantly face to face with telephone consultations when required.
  • Ability to conduct remote consultations/assessments
  • Preferred skill experience in Emis.

Main duties of the job

  • Patient Assessment:Conducting thorough physical exams, taking medical histories, and ordering diagnostic tests.
  • Diagnosis and Treatment:Making independent diagnoses, developing treatment plans, and prescribing medications within their scope.
  • Chronic Disease Management:Managing chronic conditions like diabetes and COPD, working with patients to achieve optimal health.
  • Acute Care:Providing care for acute presentations and minor health problems.
  • Referrals:Making appropriate referrals to specialists or organizing emergency admissions when needed.
  • Investigations:Ordering necessary blood tests, radiology, and other investigations.

The Advanced Nurse Practitioner(ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

b.Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

c.Implement and evaluate individual specialised treatment plans for chronic disease patients

d.Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

e.Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence.

f.Undertake the collection of pathological specimens

g.Request pathology services as necessary

h.Process and interpret pathology and other test results as required

i.Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

j.Maintain accurate clinical records in conjunction with extant legislation

k.Ensure SNOMED CT codes are used effectively

l.Maintain chronic disease registers

m.Chaperone patients where necessary

n.Prioritise health issues and intervene appropriately

o.Support the team in dealing with clinical emergencies

p.Recognise, assess and refer patients presenting with mental health needs

q.Be an extended and supplementary prescriber, adhering to extant guidance

r.Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

s.Contribute to practice targets (QOF, etc.), complying with local and regional guidance

t.Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

u.Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

v.Support the clinical team with all safeguarding matters in accordance with local and national policies
