Job summary
The post holder will be an Advanced Nurse practitioner who can demonstrate the following:-
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Post registration experience in at least one of the following areas, relating to the service applying for: Out of Hours Urgent Primary care, General practice, walk in centre, A&E.
- Be able to see patients of all ages
- Willing to undertake predominantly face to face with telephone consultations when required.
- Ability to conduct remote consultations/assessments
- Preferred skill experience in Emis.
Main duties of the job
- Patient Assessment:Conducting thorough physical exams, taking medical histories, and ordering diagnostic tests.
- Diagnosis and Treatment:Making independent diagnoses, developing treatment plans, and prescribing medications within their scope.
- Chronic Disease Management:Managing chronic conditions like diabetes and COPD, working with patients to achieve optimal health.
- Acute Care:Providing care for acute presentations and minor health problems.
- Referrals:Making appropriate referrals to specialists or organizing emergency admissions when needed.
- Investigations:Ordering necessary blood tests, radiology, and other investigations.
