Job summary

We are looking for a confident individual that can join our supportive and forward-thinking practice. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients and to support the delivery of patient care using practice policy and procedures. This is a permanent full time position however hours are negotiable for the right candidate. There is potential for development to progress to a leadership and management role within the nursing team in the future.

Main duties of the job

You will work as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population

The core responsibilities are:

  • Implement and evaluate treatment plans for Chronic Disease and maintain chronic disease registers
  • Process patient blood results as required
  • Assist other members of the nursing team

About us

Holland House Surgery is set across two sites:- Lytham Primary Health Centre and Freckleton Health Centre. It is a busy practice which is made up of 6 partners, supported by 2 salaried GP's, 2 Advanced Nurse Practitioners, 2 pharmacists and Clinical practitioner. There are currently 3 Practice Nurses, 1 Advanced Practitioner and 2 Health care assistants. We also have a host ofnon-clinical staff that form the foundation of our practice.

Holland House strives to provide an exemplary primary health care service for all patients registered with the practice making use of all the services available. We are a supportive practice that encourages development and participation in improving services.

Job description

Job responsibilities

Clinical responsibilities:

  • In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, paperwork and correspondence in a timely fashion, complete QOF requirements for patients
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • In consultation with patients and in line with current practice disease management protocols, developing health care plans for patients
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Collecting data for audit purposes
  • Implement vaccination programmes for adults and children
  • Undertake appropriate home visits (own vehicle required)
  • Adhere to Infection Control Policies
  • Review and action patient blood results advising accordingly
  • Support the team in dealing with clinical emergencies
  • Chaperone patients where necessary
  • Liaise with multidisciplinary team when there are safeguarding concerns and with external agencies to ensure that vulnerable patients are supported appropriately.

Other responsibilities within the organisation:

  • To record all clinical activity and health promotion data in the Practice computer system
  • Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety and infection control
  • Contributing to evaluation/audit and clinical standard setting within the organisation
  • Contributing to the summarising of patient records and read-coding patient data
  • To maintain full accurate and contemporaneous records of activities both written and computerised as applicable to an agreed standard. The practice has a paperless approach and as such the computer records form the main record of all interactions with patients
  • Attending training and events organised by the practice or other agencies, where appropriate.
  • To work with multi-disciplinary team within the practice, and across the wider ICB to promote integrated and seamless pathways of care
  • To maintain good lines of communication with the partners, practice management team and wider multi-disciplinary and administrative team
  • To promptly notify the Practice Manager or her deputy of any absence
  • To observe Health and Safety rules as outlined in the Practice Health and Safety Policy
  • To maintain standards of conduct and dress to maintain public confidence
  • Participate in annual performance appraisals
  • Participate in review of significant event analysis
