We are offering an exciting opportunity to join Blackpool North Primary Care Network as a PCN Clinical Pharmacist as an Independent prescriber, to work alongside our current team of Clinical Pharmacists within both surgeries.

We recognize the value that pharmacists can bring to our practices and our patients and we look forward to growing our PCN pharmacy team. The Clinical Pharmacy team is integrated within the general practice teams , working to improve the outcomes and values from medicines. The role involves working as a part of a multi-disciplinary team and is pivotal in improving the quality of care and ensuring patient safety.

This is a fixed term position, for the duration of the PCN.

Main duties of the job

As a Clinical Pharmacist, the successful candidate will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex needs. In addition, the role requires provision of support to general practice staff with regards to precription and medication queries. Specifically, this will include supporting the repeat prescription systems, dealing with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in both GP practices.

Blackpool North Primary Care Network (PCN) comprises of Glenroyd Medical and North Shore Surgery. As a PCN we work closely, to support our patients in streamlining and creating proactive and personalised care, serving a population of approximately 28,000.

Job responsibilities

Direct Patient Facing Working

Conduct face-to-face, telephone, domiciliary and virtual (where appropriate) clinical medication reviews to proactively manage patients with chronic conditions.

Proactively identify and manage patients with complex polypharmacy

Review the ongoing need for each medication, ensuring monitoring needs are performed and an opportunity to support patient achieve better health outcomes.

Reduce inappropriate poly-pharmacy and wasteful prescribing.

Make appropriate recommendations.

Medicines Information

Answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams and patient with queries about medicines.

Suggest and recommend solutions using clinical knowledge and trusted pharmaceutical sources.

Medicines Reconciliation

Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.

To review medications for newly registered patients.

Medicines Management

Produce and implement a practice repeat prescribing policy helping to improve safety and efficiency.

Develop and implement ways to improve monitoring of high-risk medication such as anticoagulants, anticonvulsants and DMARDS etc.

Monitor practice prescribing against local guidelines and formulary recommendations.

Develop prescribing guidelines for use within the Network.

Help increase uptake of Electronic Repeat Dispensing

Medicine Safety

Implement changes to medication that result from CAS/MHRA alerts, product withdrawal and other local/national guidance.

Develop and implement systems to identify cohorts of patients most likely to be at risk of unplanned admissions and readmissions from medication.

Collaborative Working Relationships

Work collaboratively with the PCN team

Develop relationships and work closely with community, hospital, and pharmacy colleagues and the wider health system.

Liaise with other stakeholders for collective benefit of patients

Attend relevant network and practice meetings.

Education and Training

To analyse latest prescribing information and trends and subsequently present this in an appropriate format to clinicians within practices to promote good prescribing practice.

Provide specialist education to pharmacy colleagues, nursing staff and GPs on aspects of medicines optimization.

Demonstrate self-development through continuous professional development.

Implement strategies to support antimicrobial stewardship.

Network Targets

Support practices to deliver on QIPP agenda, QOF, IIF and locally commissioned enhanced services.

To be aware of prescribing budget and identify any action areas for cost improvement.

Work with PCN/Practice team to ensure practices are compliant with CQC standards where medicines are involved.
