Job summary

As Digital and Transformation lead you would be expected to support increased access to care for patients, by supporting the adoption and/or optimisation of new technology and other initiatives to improve the care offer and enabling PCN staff to work more effectively to support the sustainability of general practice services.

Main duties of the job

Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity

Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access)

Lead an improvement approach to change including building capability for quality improvement within the PCN and system wide approaches to problem solving

Review and improve the PCNs digital maturity

Use data, and improve data quality, to:

o Understand demand, capacity and activity

o Hit government set targets

o Drive improvements in patient experiences of access and staff experience at work.

o Support population health management

o Support understanding of the type and intensity of support/training needs of the PCN and coordinate this support, including through OD programmes

o facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level

o to improve access to services for patients.

Blackpool North PCN consists of Glenroyd Medical and North Shore Surgery. As a PCN we work closely, to support both our patients in streamlining and creating proactive and personalised care, serving a population of approximately 28,000.

You will support a highly skilled clinical team consisting of Advanced Clinical Practitioners, Clinical Pharmacists, First Contact Practitioners and designated Visiting Team.

Job responsibilities

Key responsibilities:

Project Management

Deliver significant project that support the PCN and lead to improvements in health outcomes for the patient population.

Manage the whole lifecycle of a programme of system change.

Support organisational change and the uptake of initiative that support and embed excellence within the health system.

Manage the set up and delivery of multiple projects within a programme of work, working with colleagues and subject matter experts to scope, plan design and deliver those projects.

Review project reports and risk logs for projects, advising individual project managers on escalation process and supporting delivery of projects where there may be significant barriers to delivery.

Manage the whole lifecycle of a programme of work containing multiple projects, reporting on progress and managing risks and issues, escalating where necessary to the project board.

Provide expertise of best practice change and project management methodologies.

Ensure realisation of benefits for customers through planning and designing and that project outputs meet programme requirements.

Support organisational change and the uptake of initiatives that support excellence.

Demonstrate flexibility and support new programmes and projects where necessary.

Service improvements

Responsible for the delivery of, robust, information systems to ensure that all project documentation is stored and used effectively (PID, dashboards, highlight reports, risk and issues logs etc).

Develop programme and project management tools, report and templates that are transferable between projects and can be used for the whole project lifetime, from design to delivery.

To act as a champion for patients and their interests and involve the public and patients in the policy development and decision-making.

To ensure all public and patient contact with the office is of highest professional standard.

To embed patient and public involvement at all levels of decision making.

Be a champion for best practice, learning from experience and from others, supporting the spread of innovation within the organisation.

Analysis and Judgement

Interrogate and analyse, sometimes highly complex, information to support the delivery of clear project reporting and project outcomes.

Write and submit regular high-level project reports for the programme board showing progress against milestones, status, resource requirements, issues, risks and dependencies, making recommendations where a range of options may exist.

Develop key performance indicators and milestones for specific programmes and projects, managing conflicts and ensuring that interdependencies are managed effectively.

Communication

Communicate, engage and build relationships with key strategic policy makers to influence the development of strategy and policies.

Engage and manage relationships with stakeholders to achieve a common purpose and realise mutual benefits.

Communicate the vision for the PCN and support the development of strategy and operational policies that support this vision.

Forge close working relationships across the PCN to facilitate an effective matrix style approach to delivery of projects and programmes of work.
