We are seeking applicants who are clinically experienced, motivated, enthusiastic, and able to work autonomously and safely within the limits of their own competence and enjoy working collaboratively within a team.

The roles main focus will be to provide high quality, planned and proactive care for care home patients and to avoid unnecessary hospital admissions.

They will ensure that the Enhanced Care in Care Homes Framework and Primary Care Network (PCN) Enhanced Health in Care Homes Directed Enhanced Service (DES) is translated into the provision of evidenced based practice within Care Homes and scope of own practice. The Care Home Nurse will build on and strengthen the link between all health and care professionals delivering care and support to care home residents inclusive of colleagues within General Practice and Community Services, Social Care, Care Home Teams, and the Voluntary Care Sector.

The Nurse will work collaboratively and proactively within a multi-disciplinary team (MDT) to ensure care and support for care home residents is seamless and timely.
