Job summary
This role is a PCN role, 12 month contracts are confirmed in March of each year.
To be responsible for
implementing an effective medicine management system within the practice,
including cost-effective prescribing, working as an independent prescriber,
providing advice and support for both patients and colleagues. In addition, the
post-holder will also support the multi-disciplinary team, in line with the
strategic objectives of the practice.
Main duties of the job
1) Practice
as an independent prescriber working within scope of practice.
2)To provide
medication reviews to patients (telephone and face-to face)
3) To provide
advice and support for patients with self-limiting conditions
4) Encourage
cost-effective prescribing within the practice
5)Review all
safety alerts ensuring the required actions are taken
6)To
reconcile medicines following patients discharge from hospital or clinic
appointments
7)Review
medications for newly registered patients, ensuring that all work is completed
in a timely manner.
About us
The Delamere Medical Practice is a large surgery with 15 thousand patients, we have 5 GP partners, 4 Salaried GPS, 1 PA, 3 Practice Nurses, 1 Care Coordinator, 1 FCP and 4 GPAs. We also have a large admin and reception team.
