Job summary

This position is to specifically care for patients who have been discharged from hospital but are not ready to be discharged home, some of the patients do return home and others become permanent residents.

Patients are registered with us on a temporary basis and usually for no longer than 6 weeks at a time.

CBU (Challenging Behaviour) patients are registered with us on a permanent basis as they tend to be at the homes for longer periods of time and generally become permanent residents.

* To Provide three, weekly ward rounds to each of the Nursing Homes

* To liase with staff and family of the residents

* To attend MDT meetings

* To provide additional visits for patients as and when needed

* To be available and monitor Emails until 4pm on each working day

* To respond to queries quickly and efficiently

Job responsibilities

The Delamere Medical Practice have been looking after DTA and CBU patients for the Trafford Area since July 2022. We support a number of Nursing Homes who have beds available for DTA (Discharge to Assess) and CBU (Challenging Behaviour) Patients. These patients are primarily looked after in two nursing homes but there are spot beds in several other homes based around Urmston, Stretford, Flixton and Altrincham.

We have a dedicated Email Address and contact number which is looked after by admin support but does need to be monitored until 4pm daily, we receive a lot of last-minute medication requests and queries which often need to be dealt with on the day.

Although this position requires set days it also offers a degree of flexibility. Admin work can be completed on site at the surgery or remotely as a laptop will be provided. Ward rounds can be carried out any time during the working day.
