Job summary

We are looking for an enthusiastic and patient focused Practice Nurse to join our friendly multidisciplinary team 3 day's a week. Must be able to work a Monday and Thursday and either a Tuesday or Wednesday.

The role offers a varied and rewarding workload, including chronic disease management, health promotion, vaccinations and evidence based nursing care, with opportunities to contribute to service development and support colleagues. Main duties of the job

Develop, implement and embed health promotion, wellbeing and vaccination programmes for adults and children.

Deliver evidencebased nursing care in line with NICE, NSF and national guidance, including wound care, travel medicine, chronic disease management and routine nursing interventions.

Assess, implement and evaluate individual treatment plans, maintaining chronic disease registers and accurate clinical records with effective use of coding.

Identify, manage and support patients at risk of long term conditions, prioritising health needs, preventing deterioration and referring to secondary or specialist care where appropriate.

Recognise, assess and refer patients with mental health needs, support patients with prescribed and overthecounter medicines within scope of practice, and provide opportunistic health promotion.

Request pathology services as required, assist GPs with minor surgery, chaperone patients when necessary, and support the clinical team during emergencies.

Liaise with external services to ensure appropriate support for vulnerable patients and contribute to safeguarding in line with local and national policy.

Delegate clinical responsibilities safely, support and mentor junior nursing staff, participate in shared learning and local initiatives, and continually review and adapt clinical practice in response to national policies and service developments.

Essential

Desirable

  • Experience working in primary care or general practice, with training or competence in chronic disease management, immunisations and cervical screening.
