Job Title: Care Navigator
Location: Kiveton Park
Overview of Role:
As a care navigator, you will be responsible for receiving, assisting, and directing patients in accessing the appropriate service or healthcare professional in a courteous, efficient, and effective manner. You will provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
Main Areas of Responsibility:
- Ensure an effective and efficient reception service is provided to patients and visitors.
- Care navigate patients to the correct clinician when arranging appointments.
- Deal with general enquiries, explain procedures, and make new and follow-up appointments.
- Explain practice arrangements and formal requirements to new patients and those seeking temporary cover.
- Manage telephone calls, divert calls, and take messages with accuracy and prompt delivery.
- Assist patients with the NHS app.
- Enter requests for home visits into EMIS Web, ensuring careful recording of all relevant details.
- Action repeat prescription requests.
- Fill specialised clinics (asthma, COPD, cervical screening, diabetes, CVD).
- Advise patients of relevant charges for private services, accept payment, and issue receipts.
- Update patient information on the computer, including changes of address and telephone numbers.
- Retrieve and re-file records, ensuring strict alphabetical order.
- Ensure correspondence, reports, and results are scanned and dated promptly in the correct records.
- Register new patients onto the computer system promptly and accurately.
- Undertake additional duties as requested by the Partners or Practice Manager.
- Inform waiting patients if doctors are overrunning.
- Attend in-house training events and the Rotherham-wide meeting annually.
- Process insurance/claim forms and inform patients of processing time and charges.
- Cover for colleagues on annual/sick leave.
- Stay updated with CQC, Safeguarding, and confidentiality requirements.
- Complete annual IT training.
- Manage GP2GP and GP Links.
- Remove deceased patients from the practice list and send notes to the health authority.
Confidentiality:
- Maintain strict confidentiality regarding patient information and practice business.
- Only divulge information to authorised persons in accordance with practice policies.

- Promote and maintain health, safety, and security as defined in practice policies.
- Use personal security systems and identify risks in work activities.
- Participate in infection control training and report potential risks.
