You have a unique opportunity to join our high achieving, forward
thinking Nursing Team in shaping the future of primary care. We are a friendly,
busy, growing GMS practice in Melksham, working from a purpose built Medical
Centre. We have a multi skilled team with a wealth of experience and are
looking to welcome another Practice Nurse with treatment room skills and
chronic disease experience, if possible, to our team.
The successful candidate will need to be flexible to fit in with
the needs of the business, staff sickness and annual leave cover.
Main duties of the job
Working in accordance with the NMC Code of
Conduct, the post-holder will provide assessment, treatment, screening, health
education services and advice to patients.
The post-holder will work with reference to Practice
agreed protocols and guidelines, providing nursing treatments to patients both
independently and in participation with the Nurse Manager, Nurse colleagues and
the GP Partners.
The post-holder will
have the required level of training and competence and will work within patient
group directions where appropriate, following practice policy, Meds Mgmt, NICE
guidance and current legislation.

You will be fully supported by a broad clinical team comprising of GPs, Clinical Pharmacists, Practice Nurses, Advanced Nurse Practitioner and Health Care Assistants. It is a permanent role. Assess, plan, develop, implement and evaluate
treatment programmes that promote health and wellbeing
Assess, plan, implement and evaluate individual
treatment plans for patients with a known long term condition
Proactively identify, diagnose and manage
treatment plans for patients at risk of developing a long term condition as
appropriate
Work with other health care professionals to
diagnose, monitor, manage and treat long term conditions including non-drug
based treatments methods using a management plan, and in line with national and
local policies and Practice needs
Review medication and therapeutic effectiveness,
appropriate to patient needs and in accordance with evidence-based Practice and
national and Practice protocols
Work with patients in order to support adherence
to prescribed treatments
Have a good knowledge of safeguarding policies
in relation to both children and adults
Work to maintain and update disease register
proactively ensure patients attend appointment
Audit and work on findings re patients who do
not attend for chronic disease management
Provide information on long term conditions
Managing and taking action on blood results

Work with the administration team to co-ordinate
the QOF recall system for Chronic Disease
Be responsible for achieving QOF targets
directly linked to Chronic Disease
Support patients to adopt health promotion
strategies that promote patients to live healthily, and apply principles of
care
Managing the long term conditions of patients

Ensure that any nursing colleagues who are
actively involved in the monitoring of any Chronic Disease are working to their
potential and have a good understanding of the requirements of the disease
Maintaining infection control standards to
minimise the spread of infections among patients and staff including
preventative measures, such as hand washing, cleaning, disinfecting and
sterilising equipment and partaking in associated infections control audits
