Job summary

An exciting opportunity has arisen for a dedicated and enthusiastic Paramedic to join our growing team of multi-disciplinary clinicians delivering care to patients within the Anfield & Everton Primary Care Network.  We our currently team of PCN pharmacists, pharmacy technician, Community Mental Health Worker, Social Prescriber and Care - co-ordinators.  Our aim is to build a team of paramedics to join our MDT improving the health outcomes of our patients.

Main duties of the job

As a Paramedic you will be required to work autonomously across the practices, providing a range of services such as assessment, diagnosis, treatment, telephone triage, clinical decision-making and referring patients appropriately. You will support clinicians in the management of patients, and the Home Visiting Service; taking notes, examining patients, performing appropriate procedures, and communicating with relatives.  We would expect the post holder to continue their CPD to level 7 if they have not already attained this.

We hold a wealth of experience of caring for our practice population with long standing GP's who have cared for patients in this area for many years. As a team of practices we have embraced the new contracts developing our existing team and building new relationships with the practices. Job responsibilities

The Paramedic will be required to work autonomously in the practice, providing a range of services such as assessment, diagnosis, treatment, telephone triage, and clinical decision-making and referring patients appropriately. To support clinicians in the management of Care Home patients, and Home Visiting Service, taking notes, examining patients, performing appropriate procedures, communicating with relatives by taking part in ward rounds of care homes. Work as part of the Acute Home Visiting team but with close collaborative working with other members of the Multidisciplinary Team and support from GPs. To work within the community, as an autonomous, accountable Paramedic. To provide holistic care for individuals including assessment, diagnosis and management. To refer and/or signpost patients in their own home or care home to other allied health professionals for example community COPD nurses, Social prescribers etc. The post holder will use advanced clinical and communication skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment. Care occasionally may need to be provided in other settings as required. Participate in quality improvement and innovations, e.g. audit, significant event analysis and development of protocols and new services. Recognise opportunities to reduce inequalities and unwarranted variation in health care for the practice population. Play an active role in in-house practice and clinical meetings. Participate in the education and training of staff and students of all disciplines Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring CPD requirements are met Complete statutory training / updating and mandatory training as defined by the practice. Take responsibility for own development, learning and performance. Identify gaps in knowledge and skills and how further learning can enhance delivery of the service.

Qualifications

Essential

  • Active and relevant professional registration with HCPC
  • Evidence of relevant CPD

Desirable

  • BSc in a training programme approved by the College of Paramedics
  • Masters degree in Advanced Clinical Practice or equivalent
  • Advanced Life Support Provider
  • Independent Non-Medical Prescribing Qualification
