Job summary
Anfield and Everton PCN are thrilled to announce that we have vacancies for pharmacists to join our PCN pharmacy team.Our PCN is made up of 6 practices across Anfield and Everton with a combined registered population of just under 40,000. We recognise and understand that in order to gain the most benefit from our pharmacy team we should assign each member a proportion of the PCNs practices, rather than attempting to provide services to all 6 surgeries. The team is led by our ARRS Lead who will provide all the support and mentorship you may need.
Through these roles you will work as part of a multi-disciplinary team, in a patient facing role, using your knowledge of medicines. You will perform clinical medication reviews and provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients across the PCN. Your role will help to ensure that patients get the best effect from their medicines, reduce waste and promote self-care. You will strive to support further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload. These roles have the potential to significantly improve quality of care and safety for the patients of our PCN.
Main duties of the job
The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. The post holder will work within their clinical competencies as part of a multi-disciplinary team to:
Provide expertise in clinical medicines management
Complete patient facing structured medication reviews, with patient safety as a key priority
Manage medicines on transfer of care
