Job summary

Riverport Medical Practice is looking to appoint an experienced clinician to join our team for 6-8 sessions per week.

The ideal candidate will be an independent prescriber, and have experience working in General Practice as an Advanced Nurse Practitioner or Advanced Clinical Practitioner.

The successful candidate will have achieved, or be working towards, a Masters in Advanced Clinical Practice, or equivalent qualification.

Main duties of the job

The Advanced Clinical Practitioner (ACP) will act autonomously, within their professional scope of practice, providing care for patients face to face, via telephone consultations, and home visits where appropriate.

The post-holder will operate within their own sphere of competence, and agreed acceptable limits of practice, to provide expert professional clinical advice to patients, carers, and colleagues.

The Advanced Clinical Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.

The successful candidate will use skills, knowledge and competencies, as a qualified Advanced Clinical Practitioner, in order to be responsible and accountable for managing patient care.

About us

Riverport Medical Practice is a GMS practice with 13,750 patients. Currently rated 'Good' by CQC, the Practice has three sites in St Ives, Somersham and Fenstanton.

The Practice uses SystmOne clinical system as well as other digital healthcare solutions such as AccuRx.

Riverport Medical Practice works in collaboration with the St Ives Primary Care Network to provide services to patients across St Ives and the surrounding villages.

Job description

Job responsibilities

  • Work as part of a multi-disciplinary team to deliver safe and high quality care.
  • Triage and treat patients, making referrals as necessary to other members of the primary and secondary health care teams as appropriate.
  • Co-ordinate the planning and delivery of care, demonstrating good shared-decision making.
  • Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
  • Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
  • Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
  • File incoming clinical correspondence to the Practice, and action subsequent tasks or requests as appropriate.
  • Participate in the care home ward rounds.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Ensure clinical systems kept up to date, recording and/or amending accurate details.
  • Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice.
  • Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking.
