St Johns House Medical Centre, Worcester is looking for a qualified or trainee advanced clinical practitioner (ACP) to join our practice team to help shape our future growth. We have strong connections to our PCN, with a great multi-generational patient population.

are currently/soon to be engaged with e-portfolio supported route for Advanced
Practice or in 2nd year of ACP, we would love to hear from you.
The Practice
List size of 13,800
An innovative and supportive team, comprising of two partners, six GPs, two trainee ACPs, two practice pharmacists, three nurses, two HCAs, a practice manager, an assistant practice manager, reception managers, receptionists, administrators and secretaries.
The Practice is consistently benchmarked as high performing in ICB and PCN contracts across a range of indicators, including referrals and prescribing.
Bi-weekly MDT with social and community care.

Main duties of the job
- As an ACP you'll be treating patients face-to-face or via telephone appointments.
- Work to advise and recommend treatment for patients using clinical judgment, and decision-making skills in accordance with agreed local, regional and national policies and guidelines.
- Recognise and work within own competency and professional code of conduct as regulated by the NMC/HCPC
- In partnership with other clinical staff, collaborate on improving the quality of healthcare responding to local and national policies and initiative as appropriate
- Support and participate in shared learning across the practice
- Produce accurate, contemporaneous and complete records of patient consultation
- Assess effectiveness of care delivery through self and peer review, benchmarking and formal audit and evaluation in the quality governance systems and processes of the practice.
- Prioritise own workload and ensure effective time management strategies are embedded within the culture of the team.
- Work effectively with others.
- The role will involve but not be limited to you performing the above.

- The
post-holder is expected to take due note of and act in accordance with the
standards of clinical governance that are agreed within the practice. Any
concerns should be brought to the attention of the partners as soon as
practicable.
- Act

- Organise
the clinical management of individual patients, using highly developed
expert clinical knowledge and advanced skills to assess, diagnose, plan,
implement and initiate changes in management where required. This will
involve the monitoring and evaluating the response to treatment through
analysis and interpretation of clinical tests and investigations and the
transfer of care/discharge of patients.
- Conduct
advanced health assessments and clinical examination of patients with
highly complex presentations and make clinical decisions based on these
assessments.
- Work
as an autonomous practitioner, management and prioritising own workload on
a daily basis.
- Work
to advise and recommend treatment for patients using clinical judgment,
and decision making skills in accordance with agreed local, regional and
national policies and guidelines.
- In
partnership with other clinical teams, collaborate on improving the
quality of healthcare responding to local and national policies and

- BSc (Hons) Degree in Nursing/Pharmacy/Paramedic Science.
- Registration with the relevant professional body; Nursing & Midwifery Council, General Pharmaceutical Council; Health & Care Professions Council.
- Meet the legal prescribing standard to be able to train as a non-medical prescriber.
- Advanced Life Support.
Desirable
- MSc Advanced Clinical Practice (or equivalent clinical masters degree which includes modules in advanced clinical assessment & pathophysiology).
- V300 independent prescribing qualification.
Experience
